Freeman-Sheldon syndrome (FSS) is characterized by multiple joint contractures, characteristic facial features, such as microtia, defects of the hands and feet, such as clubfoot, and skeletal malformations. This report illustrates the case of a patient with FSS who was managed under local anesthesia with intravenous sedation for oral surgery.

To the Editor.— The report by Yaster et al, “Midazolam-Fentanyl Intravenous Sedation in Children: Case Report of Respiratory Arrest,”1 is the first we are aware of in the pediatric literature which reviews and promotes the use of this method of sedation and analgesia for children. Unfortunately,the title of the article and some of its emphasis may have the unfortunate consequence of delaying acceptance of this generally safe technique which is clearly not the intent

We report a case of a 55-year-old male patient who presented with swelling over his right elbow of 5-year duration. An ultrasound examination revealed an echogenic mass within the olecranon bursa. On magnetic resonance imaging, the mass revealed the signal intensity of fat, and it was attached to the bursal lining by means of a pedicle. There was no restriction on diffusion-weighted images and no significant contrast enhancement. Excision was performed under local anesthesia. The excised specimen revealed mature fat cells on histopathology.

The article presents the material devoted to the actual problem-rehabilitation of patients with otosclerosis with sensorineural hearing loss (SNHL) IV degree and deafness with concomitant diseases. Material and methods: on the basis of Clinical Research Center of Otorhinolaryngology in the Department of ear diseases cochlear implantation (CI) was performed in 4 patients with otosclerosis and comorbidities under local infiltration anesthesia and intravenous sedation with Dexdor drug, which allowed to avoid the introduction of muscle relaxants. In the preoperative period, patients were carefully prepared for the operation, acquainted with each stage of the operation, with tables for intraoperative dialogue. CI was performed by the classical method. Each patient was also interviewed for all parameters (during surgery and in the early postoperative period). Results. After parenteral administration of the drug, the effect was achieved quickly enough, against the background of drug administration, blood pressure did not increase to high figures relative to the norm, aggravation of comorbidities was not noted, patients felt satisfactory, responded to all signs, answered questions by reading from the tables. In no case did patients feel pain during incision, soft tissue separation, boron machine operation, introduction of cochlear implant electrode array and its subsequent testing. Conclusions. The advantages of local anesthesia using the Dexdor drug include: less invasiveness of the procedure; the ability not to use muscle relaxants; cost savings; finding the patient in consciousness, which makes it possible to intraoperatively diagnose the implant and assess the patient’s auditory perception when giving signals; determine the presence or absence of pathological stimulation of the facial nerve; reducing the time of surgery.

Enchondromas are benign tumours that may become symptomatic due to expansive pressure on the surrounding bone. In this case, a 27-year-old male developed a symptomatic enchondroma within the proximal phalanx of his left fourth toe. Resection and insertion of a bone graft were considered optimal management. Histopathology testing confirmed the diagnosis of an isolated enchondroma. The patient was monitored closely for 3 months postoperatively and reported full satisfaction at his 12-month review. Enchondroma resection and insertion of a tibial bone graft may provide an effective long-term solution for a symptomatic enchondroma of the toe.
