Perceptual Characteristics of Consonant Production by Apraxic Speakers

Single-word repetitions by 4 brain-damaged adults with apraxia of speech (AOS) but without concomitant aphasia were transcribed using a standard narrow phonetic transcription system. Analysis of consonant productions yielded a profile of AOS slightly different from the traditionally accepted one. Among the results was the atypical finding that consonant distortions exceeded all other error types including sound substitutions. In addition, errors predominated in the medial position of words, and monosyllabic words had approximately the same error rate per number of consonants as did multisyllabic words. Results are discussed with reference to previous perceptual descriptions of AOS and in relationship to Broca's aphasia. Results are also interpreted relative to linguistic and motoric components of speech production models.

Narrow phonetic transcriptions and prosodic judgments were made of single-word imitations by apraxic (AOS), conduction aphasic (CA), and ataxic dysarthric (AD) speakers. AOS and AD subjects showed similar vowel error patterns, particularly predominant errors in low, tense, and back vowels, more distortions than other types of vowel errors, and predominant errors in initial position of words and in monosyllabic words. The CA subjects displayed a different vowel error pattern, notably more substitutions than distortions, more errors in polysyllabic than monosyllabic words, and more errors in noninitial than initial positions of words. Analysis of prosodic features identifiable at the single-word level (e.g., syllabic stress, juncture, and struggles to initiate or complete productions) indicated that syllabic stress errors and more difficulty initiating than completing word production were characteristic of AOS and AD but not CA subjects.

Purpose This retrospective analysis examined group differences in error rate across 4 contextual variables (clusters vs. singletons, syllable position, number of syllables, and articulatory phonetic features) in adults with apraxia of speech (AOS) and adults with aphasia only. Group differences in the distribution of error type across contextual variables were also examined. Method Ten individuals with acquired AOS and aphasia and 11 individuals with aphasia participated in this study. In the context of a 2-group experimental design, the influence of 4 contextual variables on error rate and error type distribution was examined via repetition of 29 multisyllabic words. Error rates were analyzed using Bayesian methods, whereas distribution of error type was examined via descriptive statistics. Results There were 4 findings of robust differences between the 2 groups. These differences were found for syllable position, number of syllables, manner of articulation, and voicing. Group differences were less robust for clusters versus singletons and place of articulation. Results of error type distribution show a high proportion of distortion and substitution errors in speakers with AOS and a high proportion of substitution and omission errors in speakers with aphasia. Conclusion Findings add to the continued effort to improve the understanding and assessment of AOS and aphasia. Several contextual variables more consistently influenced breakdown in participants with AOS compared to participants with aphasia and should be considered during the diagnostic process. Traditionally, speech-language pathologists (SLP) have been trained to develop interventions based on a select number of perceptual characteristics of speech without or through minimal use of objective instrumental and physiologic assessment measures of the underlying articulatory subsystems. While indirect physiological assumptions can be made from perceptual assessment measures, the validity and reliability of those assumptions are tenuous at best. Considering that neurological damage will result in various degrees of aberrant speech physiology, the need for physiologic assessments appears highly warranted. In this context, do existing physiological measures found in the research literature have sufficient diagnostic resolution to provide distinct and differential data within and between etiological classifications of speech disorders and versus healthy controls? The goals of this paper are (a) to describe various physiological and movement-related techniques available to objectively study various dysarthrias and speech production disorders and (b) to develop an appreciation for the need for increased systematic research to better define physiologic features of dysarthria and speech production disorders and their relation to know perceptual characteristics.

Twenty children who have worn a Cochlear Corporation cochlear implant for an average of 33.6 months participated in a device-on/off experiment. They spoke 14 monosyllabic words three times each after having not worn their cochlear implant speech processors for several hours. They then spoke the same speech sample again with their cochlear implants turned on. The utterances were phonetically transcribed by speech-language pathologists. On average, no difference between speaking conditions on indices of vowel height, vowel place, initial consonant place, initial consonant voicing, or final consonant voicing was found. Comparisons based on a narrow transcription of the speech samples revealed no difference between the two speaking conditions. Children who were more intelligible were no more likely to show a degradation in their speech production in the device-off condition than children who were less intelligible. In the device-on condition, children sometimes nasalized their vowels and inappropriately aspirated their consonants. Their tendency to nasalize vowels and aspirate initial consonants might reflect an attempt to increase proprioceptive feedback, which would provide them with a greater awareness of their speaking behavior.

The speech of a five-year-old boy who suffered a profound hearing loss following meningitis was sampled at two-week intervals for nine months. Speech samples were subjected to phonetic transcription, spectrographic analysis, and intelligibility testing. Immediately post-trauma, the child displayed slightly slower, F o elevated, acoustically intense speech in which phonemic distortion and syllabification of consonants occurred occasionally; single word intelligibility was depressed below normal between 20–30%. By the 18th week, a sudden decline in intelligibility, increasing monotony of pitch, and a pattern of strongly emphatic, prolonged, aspirated, syllabified, and increasingly distorted consonants were manifest. At year's end, the child's speech bore some resemblance to the speech of the deaf in terms of suprasegmentals, intonation, and intelligibility, but differed because the child rarely, if ever deleted speech sounds or diphthongized vowels strongly. It is speculated that phonetic processes such as diphthongization, syllabification, and prolonged duration may be strategies for enhancing feedback during speech.
