Introduction Thyroglossal duct cyst (TGDC) is the most common congenital anomaly of the neck, and approximately 7% of all the adult population presents it. Ectopic thyroid tissue is found in the thyroglossal duct cyst wall in up to 65% of cases. This thyroid tissue has the potential to develop some type of malignancy, the most common of which is the papillary carcinoma of the thyroid. There are just over 270 cases of thyroglossal duct cyst malignancy reported in the literature. Objectives We aimed to study our population of patients in order to identify cases with thyroglossal duct cyst malignancy. Methods A retrospective chart review was conducted in the section of otolaryngology/head and neck surgery at a hospital in Karachi, Pakistan, from January of 2004 to December of 2014 on patients with the diagnosis of thyroglossal duct cyst. Results Fifty-eight patients were diagnosed with TGDC, two of whom also presented with thyroglossal duct cyst carcinoma. Both patients revealed papillary thyroid carcinoma on histopathology. Case 1 had an open biopsy before undergoing definitive surgery. Both patients underwent subsequent total thyroidectomy after Sistrunk procedure, and case 2 had selective neck dissection revealing lymph node metastasis. Conclusion Thyroglossal duct cyst carcinoma is a rare finding that comes as a surprise to both the patient and the surgeon. We report 2 out of 58 cases diagnosed with thyroglossal duct cyst carcinoma.

BACKGROUND: Thyroglossal duct cysts occur in approximately 7% of the adult population. In children, it is the most common cervical congenital anomaly, with a frequency of 70%. Intracystic malignancy is very rare, it occurs in less than 1% of cases, with a mean age of 40 years and predominance in women. CASE REPORT: 29-year-old female patient, with no significant medical background. She consulted for a “lump” in the front neck, which had been slowly growing for three years, and did not produce any symptoms. Physical examination and complementary imaging tests resulted in the diagnosis of an occupational lesion compatible with a thyroglossal cyst. Sistrunk surgery was performed. EVOLUTION: During the surgical procedure, a mass was identified over the hyoid bone, macroscopically compatible with a thyroglossal duct cyst; however, the histopathology report showed papillary thyroid carcinoma within the cyst. The case was discussed; it was decided to only indicate control and follow-up. The patient has been on follow up for 30 months, without recurrence to date. CONCLUSION: Although its incidence is low, papillary thyroid carcinoma can present within a thyroglossal duct cyst; the incidence is slightly higher in female population. Diagnosis in most cases is made after surgical treatment. The treatment choice must be individualized in each case, depending on the risk of recurrence and metastasis, following the current established recommendations.

Thyroglossal duct cyst is the most common upper neck midline lesion in children. Approximately 1% of thyroglossal duct cyst undergoes malignant transformation. Calcification which is a marker of malignancy almost always points out to papillary thyroid carcinoma. Benign case of calcified thyroglossal cyst maybe undiagnosed or under reported. We report a case of a 5 year old girl with a benign calcified thyroglossal duct cyst, a fourth case in world literature. Here the differences between a benign and malignant thyroglossal duct cyst are discussed.
