Managing Risk in the Pro-Empowerment Era of Mental Health Care: A Cross-Cultural Study of Social Work Perspectives in Hong Kong and Sydney

Abstract Risk management and empowerment have become key features of social work practice. Despite their increasing salience, relatively little is known about the perspectives of mental health social workers regarding how they navigate competing risk management approaches in modern practice that supports empowerment. The socio-cultural influences on risk management have also received insufficient attention in social work research. Focusing on these issues, this paper explored the perspectives of social workers in two geographically and culturally distinctive settings (i.e., Hong Kong and Sydney). Data were collected through semi-structured interviews with frontline social workers and were analysed using thematic analysis. Similar views were expressed by participants in both settings regarding assessment of clients’ readiness for risk-taking. Differences were identified in their practices of negotiating the perspectives of other key stakeholders and can be attributed to the influences of distinctive cultural and socio-political contexts. These insights may contribute to the development of more systematic, localised and practice-based risk assessment guidelines for mental health practitioners working towards the empowerment of clients.

Abstract The international policy trend towards personalised budgets, which is designed to offer people with disabilities purchasing power to choose services that suit them, is exemplified in the Australian National Disability Insurance Scheme (NDIS). This article examines how the ‘purchasing power’ afforded to service users through individualised budgets impacts on social work practice and the choice and self-determination of NDIS service users. Social workers’ views were sought on the alignment between the NDIS principles of choice and control and social work principles of participation and self-determination and how their social work practice has changed in order to facilitate client access to supports through NDIS budgets and meaningful participation in decision-making. A survey was completed by forty-five social workers, and in-depth semi-structured interviews were conducted with five of these participants. The findings identify how social workers have responded to the shortfalls of the NDIS by the following: interpreting information for clients; assisting service users to navigate complex service provision systems; supporting clients through goal setting, decision-making and implementation of action plans; and adopting case management approaches. The incorporation of social work services into the NDIS service model is proposed in order to facilitate meaningful choice and self-determination associated with purchasing power.

BACKGROUND Information and communication technology (ICT) use may enhance social work practice and continuous professional development. Under the Hong Kong Jockey Club SMART Family-Link project, we developed an innovative web-based training, learning, and sharing (i-TLS) platform to support ICT and other learning needs of Hong Kong social workers in family services. OBJECTIVE We developed i-TLS with 3 major components (i-Training, i-Learning, and i-Sharing) and assessed the acceptability and impact on facilitating ICT use in family services. METHODS We described i-TLS development based on a 4-phase model from platform design, development, implementation to maintenance. We evaluated i-TLS via platform database, Google Analytics, a self-administered survey, and individual phone interviews 1 year after launching. RESULTS i-TLS was launched to 26 Integrated Family Service Centers (IFSCs) and Integrated Services Centers (ISCs) operated by 12 non-governmental organizations (NGOs) on 1 July 2019. The outbreak of COVID-19 started in December 2019 limited face-to-face services, which catalyzed the urgent needs of digital transformation in social work practice. By 31 July 2020, 313 social workers (23 supervisors, 290 frontline workers) had registered with i-TLS. The platform database showed 79.6% (249/313) users accessed i-TLS at least once in the last 28 days, with on average 3.2 platform visits per day viewing 4.8 pages per visit. i-Training provided 41 training mini-modules in applying ICT to family services from counseling, program design, implementation to evaluation. Of 730 enrolments in total, 70.0% (511/730) completed the mini-modules and were awarded digital mini-certificates. i-Learning provided 112 items of learning resources centered around ICT use and family services and had nearly 4000 page views recorded from Google Analytics. i-Sharing had a total of 25 discussion threads with 59 posts. 53.7% (168/313) users completed the 1-year evaluation survey, including 12 who participated in the phone interviews. The mean i-TLS satisfaction score (out of 10) increased from light (4.99) to occasional (6.15) and frequent (6.31) users. Frequent users showed higher scores (out of 10) than light users for an increase in knowledge (5.84 versus 4.09; P<.001), self-efficacy (5.23 versus 3.96; P=.02), and knowledge application (6.46 versus 1.91; P<.001). From the phone interviews, users reported increased ICT use in family services, despite some practical barriers. i-TLS was perceived as an acceptable and supportive tool for learning and practice in family services, especially during the COVID-19 pandemic. CONCLUSIONS We have first reported the development and evaluation of a newly developed web-based learning platform (i-TLS) for social workers in family services. The results provided preliminary evidence of using i-TLS to support social workers’ continuous learning and ICT-enhanced services. Accessibility to self-directed and collaborative learning is essential for optimizing social workers’ learning. Further research on enhancing web-based platforms is needed to expand participation and capacity building of social workers and other related professionals. Using an anti-oppressive practice (AOP) theoretical framework and an exploratory qualitative research design, featuring semi-structured interviews and written assignments, a group of ten social workers were asked to describe their understandings of the concept of oppression. The study found that, in the case of these particular social workers, they used metaphor as a key conceptualization process to more vividly describe and understand the concept of oppression within their social work practice. This article analyzes eight categories of metaphor themes the participants used to explain their understanding of oppression: (a) pressure; (b) earth; (c) quest; (d) nature of society;  (e) seeing; (f) building; (g) dancing; and (h) water. The research findings are intended to open up dialogue and thinking about the concept of oppression, increase our knowledge base and understandings of oppression within social work practice, and assist the social work profession to build a stronger conceptual framework for understanding and naming oppression with the end goal of assisting social workers to better respond to and resist systems of domination.

Summary Interprofessional practice has been broadly endorsed as a strategy which supports the delivery of high-quality health services. Few studies have addressed the dynamics inherent to interprofessional mental health practice in an era in which client empowerment is prioritised and valued. Social work perspectives are also underexplored in the studies which have been published on interprofessional health care, despite the increasingly important role of this discipline in community mental health contexts. Through a thematic analysis of 26 semi-structured interviews, this study explored the perspectives of mental health social workers located in two geographically and culturally distinctive sites (Hong Kong and Sydney), and identified a strong theme encompassing their concerns about the facilitation of client empowerment in interprofessional settings. Findings Participant accounts direct attention to the strategies required to co-constitute the shared territories of the different mental health professions. Findings also highlight the frustration with recent mental health reforms which aim to challenge power differentials in interprofessional decision-making. Strategies were proposed for supporting the construction of shared language among different professions, revealing the creativity and insight of practitioners in managing the conflicts arising from interactions between different professional knowledge systems. The comparison of Hong Kong and Sydney accounts highlights the importance of structural, political and cultural influences in supporting not only interprofessional collaboration but also the independence and effectiveness of distinct professional roles. Application This article provides unique insights which aim to strengthen the contribution of social work to interprofessional practice in the promotion of empowerment-oriented interprofessional practice and education.

The aim: Social work is focused on overcoming inequality and social isolation of individuals and communities due to health conditions. Therefore, substantiating the activities of social workers to preserve mental health of citizens is an important component of the new strategy for the development of the profession in the XXI century. Materials and methods: The methodological basis of the study is a holistic approach to the interpretation of individual health in the unity of physical, mental, social, and spiritual components; ecosystemic approach assumes the impact of the environment on the social functioning of the individual; interdisciplinary approach identifies ways of dealing withmental health challengein the global context; social inclusion approach provides a basis for the practice of experts of the social spherein the XXI century. Conclusions: The article substantiates the need for social workers to maintain the mental health of the population under the negative impact of global factors within their own countries and internationally. The evolution analysis of the international experience of transformations of social work practice in the area of mental health preservation is carried out. Preserving the mental health of citizens under the negative effects of globalization processes depends on the concerted actions of states, international organizations, civil society, communities, and individuals themselves. This encourages social workers to work at regional and international levels with institutions interested in fair policies and practices in social and health services.

Women constitute a growing segment of the incarcerated population across the world, presenting unique issues for social workers in correctional settings. In this article, based on a study conducted in Mandoli Jail, New Delhi, India, we address the need to broaden the scope and quality of social work provided for women inmates, specifically highlighting the potential role of social workers in promoting their mental health. Our study revealed that the mean age of women inmates was 39.43 years, indicating that most of the women were in their reproductive age and therefore prone to specific health issues. On an average, they were overweight, with only 42.22% women inmates being active, while remaining were inactive (15.56%), moderately inactive (22.22%) or moderately active (20.00%). Of greater concern were the mental health findings: 87.8% experienced distress (45.57% moderately elevated, 54.43% strongly elevated), 73.3% depression (34.85% moderately elevated, 65.15% strongly elevated), 77.78% anxiety (41.43% moderately elevated, 58.57% strongly elevated) and 82.22% somatisation (43.24% moderately elevated, 56.76% strongly elevated). Our study, consequently, identified the mental health of women inmates as a strategic priority for social workers. In this article, we explore the mutuality of various social work practice principles at different levels of intervention and the roles that social workers, as human rights-based professionals, may engage in to improve the mental health of incarcerated women.

Young migrants defined as ‘unaccompanied’ tend to be constructed as a homogeneous group with specific vulnerabilities and strengths in social work practice. ‘Unaccompanied’ young migrants placed in kinship care in Sweden are constructed with further vulnerabilities. Such constructions of these young people and their situations may have consequences for how social support for them is designed. The aim of this study is to explore how the social workers employed at a non-governmental organisation mentoring programme construct young migrants’ situations in kinship care in a Swedish suburb, and if and how these constructions change during the course of the programme. Methods used are semi-structured interviews with the social workers at the youth centre where the mentoring work takes place and analysis of the non-governmental organisation’s policy documents. The results consist of three constructions of situations the young people are in: (1) loneliness and (a lack of) support in the kinship homes; (2) alienation in the local neighbourhood and the kinship home and (3) social, cultural and family contexts creating a sense of safety. The results show variation in how the mentors describe each situation with both vulnerabilities and strengths. This highlights a complexity in the constructions that contests the image of young migrants in kinship care as merely vulnerable. These results reveal consideration of individual differences and contexts, and are used to discuss how people’s struggles and resources can be dealt with in social work.

Forensic social work can bridge the gap between the criminal justice and mental health systems and serve clients who “fall between the cracks.” The authors describe theoretical and clinical issues, utilizing case examples and the literature to develop a conceptual paradigm for the role of social workers in this area.

Substantial research demonstrates the existence of the sexual revictimization phenomenon; however, the problem of sexual revictimization is largely unattended and unexamined in the social work literature. Because of the pervasiveness of sexually violent victimization, social workers are frequently working with victim-survivors among their clients in child welfare, domestic violence, and sexual assault as well as physical health, mental health, and substance abuse settings. Social workers lacking a thorough knowledge about sexual revictimization and the strategies to prevent its occurrence may be missing important intervention opportunities. This paper helps to increase social workers’ awareness of the revictimization phenomenon and builds on the literature and research to provide recommendations for social work practice and research for revictimization prevention.
