A 49-year-old man presented with a 1-week history of right facial paraesthesia with blurred vision and diplopia. Examination was normal apart from reduced facial sensation. Following appropriate neuroimaging, we considered a diagnosis of silent sinus syndrome. He underwent a middle meatal antrostomy with complete resolution of symptoms. Silent sinus syndrome results from occlusion of the osteomeatal complex, preventing normal aeration of the maxillary sinus. Maxillary sinus hypoventilation typically causes inferior displacement of the globe in the orbit (unilateral hypoglobus). Neurologists will only infrequently see people with silent sinus syndrome but it can have devastating consequences if left untreated and so must be considered in the appropriate clinical context.

<p class="abstract">An odontogenic myxoma is a benign odontogenic tumor with locally aggressive behavior. It is relatively rare in the oral cavity. The diagnosis poses a challenge because its features overlap with those of other benign and malignant neoplasms. We present an unusual case of odontogenic myxoma with involvement of the maxilla extending to the maxillary sinus in a 17-year-old lady. A unique computed tomography (CT) appearance revealed a well circumscribed lytic lesion arising from the left maxilla causing expansion and thinning of the maxillary cortex. There is a discernible tooth with adjacent amorphous calcification in a round formation noted within the lesion. Biopsy revealed an odontogenic myxoma. The patient however did not undergo surgical excision as was not consented at the time of this write ups. This paper highlights the unique radiographic appearance of the pathology that has not been reported so far as well as to discuss its clinical significance.  </p>

We present the unusual case of a large complex odontoma erupting in the maxilla. Odontomas are benign developmental tumours of odontogenic origin. They are characterized by slow growth and nonaggressive behaviour. Complex odontomas, which erupt, are rare. They are usually asymptomatic and are identified on routine radiograph but may present with erosion into the oral cavity with subsequent cellulitis and facial asymmetry. This present paper describes the presentation and management of an erupting complex odontoma, occupying the maxillary sinus with extension to the infraorbital rim. We also discuss various surgical approaches used to access this anatomic area.
