ABSTRACT This is a case report regarding the use of non-conventional methods to perform internal cardiac defibrillation on a trauma patient in an austere environment. The patient was a polytrauma causality of an improvised explosive device who arrived to a far forward resuscitative surgical team during a recent armed conflict. After arrival, the patient lost pulses. An emergency resuscitative thoracotomy was performed, and the patient was noted to have ventricular fibrillation on direct cardiac visualization. In the absence of standard surgical defibrillation paddles, the team applied external defibrillator stickers directly to the patient’s myocardium to deliver an electrical shock. The procedure successfully led to the return of spontaneous circulation. This report highlights a novel approach to resuscitation in resource-limited environments by a military surgical team.

In this paper, a unique dataset of improvised explosive device attacks during “The Troubles” in Northern Ireland (NI) is analysed via a Hawkes process model. It is found that this past dependent model is a good fit to improvised explosive device attacks yielding key insights about the nature of terrorism in NI. We also present a novel approach to quantitatively investigate some of the sociological theory surrounding the Provisional Irish Republican Army which challenges previously held assumptions concerning changes seen in the organisation. Finally, we extend our use of the Hawkes process model by considering a multidimensional version which permits both self and mutual-excitations. This allows us to test how the Provisional Irish Republican Army responded to past improvised explosive device attacks on different geographical scales from which we find evidence for the autonomy of the organisation over the six counties of NI and Belfast. By incorporating a second dataset concerning British Security Force (BSF) interventions, the multidimensional model allows us to test counter-terrorism (CT) operations in NI where we find subsequent increases in violence.

AbstractAimTo discuss the influence of Vitamin D defi ciency in the osseointegration process of a dental implant by way of a case report.SummaryA 29-year-old soldier attended clinic with a fractured mandibular premolar (tooth 44) that was traumatised following head trauma related to the detonation of an Improvised Explosive Device (IED) whilst serving on operational duty. The tooth was deemed unsalvageable and was extracted with immediate placement of a dental implant. The patient experienced no problems but at assessment, five months post-operatively, no osseo-integration of the implant was found. Concurrent medical investigations revealed that he was severely Vitamin D deficient and that this may have contributed to the implant failure.ConclusionVitamin D deficiency may play a role in the failure of osseointegration in dental implants. The assessment of vitamin D status in patients who have been in long-term hospital care or rehabilitation should be considered, prior to the placement of dental implants.
