ABSTRACT Maxillary canine and first premolar transposition is a complicated dental anomaly to treat, especially if the clinician's goal is to orthodontically move the canine into its normal position. Early diagnosis with cone-beam computed tomography simplifies the treatment of this pathology. This case report describes a patient with bilateral transposition, one complete and the other incomplete, involving the maxillary canine and the first premolar (Mx.C.1P). The orthodontic treatment involved the correction of both transpositions. In the complete transposition, the traction was mesial and upward to move the canine into a more apical position with a wider dentoalveolar process for easier crown interchange.

This case report describes the orthodontic treatment used to prepare for restorative dentistry, aimed at the functional rehabilitation of an adult patient with Class I malocclusion, right posterior crossbite, and significant tooth wear on the palatal face of the maxillary canine to canine. The orthodontic treatment was performed with a total fixed appliance mini-expandex-type expander, associated with vertical elastics. Good dental intercuspation was obtained and enabled conditions for the restoration of the abrasions on the palatal surfaces of the maxillary incisors and canines and the tips of the buccal cusps of the maxillary right premolars and first molar. Excellent results were found during a 37-month follow-up.

ABSTRACT Transposition of maxillary teeth is an eruptive disturbance occurring in approximately 1 of every 300 orthodontic patients. Such cases are frequently very challenging in terms of treatment planning and orthodontic management. The canine is one of the most commonly transposed teeth, ectopically positioned with either the lateral incisor or the first premolar. This case report illustrates unique orthodontic treatment, describes treatment procedures, and presents the final outcome of bilateral maxillary canine-lateral incisor complete transpositions in which the involved teeth were moved to their clinically normal position in the dental arch without extracting premolars.

ABSTRACT Objective: The aim of this study was to introduce a new method for bilateral distal movement of the entire maxillary posterior segment. Case report: A 17-year-old girl with Class I skeletal malocclusion (end-to-end molar relationships, deviated midline and space deficiency for left maxillary canine) was referred for orthodontic treatment. She did not accept maxillary first premolars extraction. A modified Hyrax appliance (Dentaurum Ispringen, Germany) was used for bilateral distalization of maxillary posterior teeth simultaneously. Expansion vector was set anteroposteriorly. Posterior legs of Hyrax were welded to first maxillary molar bands. All posterior teeth on each side consolidated with a segment of 0.017 × 0.025-in stainless steel wire from the buccal side. Anterior legs of Hyrax were bent into eyelet form and attached to the anterior palate with two mini-screws (2 × 10 mm) (Jeil Medical Corporation Seoul, South Korea). Hyrax opening rate was 0.8 mm per month. Lateral cephalometric radiographs were used to evaluate the extent of distal movement. 3.5-mm distalization of posterior maxillary teeth was achieved in five months. Results: A nearly bodily distal movement without anchorage loss was obtained. Conclusion: The mini-screw-supported modified Hyrax appliance was found to be helpful for achieving en-masse distal movement of maxillary posterior teeth.
