Curative surgery for a localized tumour at the early and pre-symptomatic stage is the main aim of lung nodule assessment. As part of the process, pre-operative tissue diagnosis is a key step in making appropriate clinical decisions and avoiding unnecessary invasive surgical intervention which is associated with high morbidity. In this case report, we describe a patient who had a curative lobectomy for a growing nodule without a preoperative tissue diagnosis although the initial discussion deemed it mandatory, and we dissected the arguments supporting it in the setting of a tuberculosis endemic country.

ABSTRACT This is the case report of a postcesarean woman who presented with multiple lung secondaries, bilateral ovarian cysts and very high serum β-hCG which turned out not to be choriocarcinoma. This shows the importance of tissue diagnosis even in patients who show all other features of choriocarcinoma.

Abstract Only nine cases of solitary thalamic abscess have been reported. All were of bacterial origin, and three were successfully treated by surgical drainage and antibiotic therapy. The authors present the history of a patient with a histoplasmoma of the thalamus. As is often the case with solitary thalamic lesions, this lesion was initially assumed to be a tumor and was first treated with radiotherapy. Despite the sensitive location of the lesion, tissue diagnosis and decompression was safely accomplished with the aid of ultrasonic localization. Appropriate antifungal treatment was given, and the patient made a good recovery.

Abstract A 16-year-old boy was noted to have an apparent intradural sacral mass 6 weeks after the excision of a 4th ventricle ependymoma. Although spinal axis radiation was considered, it was decided to make a definitive tissue diagnosis. At sacral laminectomy, only segmental arachnoiditis was found. The implications of this finding are the subject of this case report.
