Granulomatous mastitis is a rare benign inflammatory condition of the breast and is known to be associated with pregnancy. A 25-year-old Hispanic G2P1 at 17 weeks gestation presented to the emergency department with findings consistent of a breast abscess. The abscess failed to resolve with incision and drainage followed by multiple courses of antibiotic therapy. A biopsy was then obtained and yielded a diagnosis of granulomatous lobulitis. The patient was treated with steroids and her symptoms resolved. Granulomatous lobulitis may present with characteristics of various clinical entities including neoplasm or, as in this case, abscess. Clinicians should consider a diagnosis of granulomatous mastitis in cases of recalcitrant breast abscess.

Abstract Idiopathic Granulomatous Mastitis (IGM) is a rare chronic inflammatory disease that involves the development of an inflammatory mass in the breast, which may be difficult to differentiate from malignancy. Few patients have been reported in the literature presenting with arthritis accompanying IGM of breast. Here we report a case of Iraqi patient presented as IGM with reactive arthritis and erythema nodosum.

Tuberculous mastitis is a granulomatous mastitis due to infection by mycobacterium tuberculosis. It’s a rare entity witch is often confused with breast carcinoma or pyogenic breast abscess because of its clinical and radiological aspect. The diagnosis is histological with identification of an epithelioid cell granulomas and necrosis. The treatment is based on anti-tuberculous therapy with a good clinical issue. Authors report a case of tubercular mastitis in a post-menopausal female diagnosed on adenectomy.

Summary Idiopathic granulomatous mastitis is a rare disorder of unknown etiology. This disease occurs mostly in young women and often after the lactation period. Women usually present with a fixed, painful mass, sparing the retroareolar region, associated with skin thickening and possible ulceration that mimics carcinoma. Nipple discharge can be present and bilateral involvement may occur in up to 25% of cases. In this case report, we present a typical case of histologically confirmed idiopathic granulomatous mastitis, highlighting the imaging findings, including magnetic resonance imaging (MRI), which may favor this diagnosis and enable better clinical management of these patients.
