Does the Acoustic Voice Quality Index (AVQI) Correlate with Perceived Creak and Strain in Normophonic Young Adult Finnish Females?

<b><i>Background:</i></b> The Acoustic Voice Quality Index (AVQI) is a correlate of dysphonia. It has been found to differentiate between dysphonic and normophonic speakers and to indicate the effects of voice therapy. This study investigates how the AVQI reacts towards creak and strain, which are common in normophonic speakers. <b><i>Methods:</i></b> The material was obtained from an earlier study on 104 Finnish female university students (mean age 24.3 years, SD 6.3 years) with no known pathology of voice or hearing and a perceptually normal voice (G = 0 in GRBAS), who were recorded while reading aloud a standard text and sustaining the vowel [a:]. Perceptual analysis for the amount of creak and strain was carried out by 2 expert listeners. In this study, the AVQI v03.01 was analyzed and correlated with perceptual evaluations. Samples with low and high amounts of creak and strain were compared with <i>t</i> tests. <b><i>Results:</i></b> On average, the AVQI was below the threshold value of dysphonia in the Finnish population. The AVQI (ρ = 0.35, <i>p</i> = 0.000) and its subparameters, smoothed cepstral peak prominence (CPPS; ρ = –0.35, <i>p</i> = 0.000) and harmonics-to-noise ratio (HNR; ρ = –0.30, <i>p</i> = 0.002) showed low but significant correlations with creak. Strain had low but significant correlations with spectral Slope (ρ = 0.38, <i>p</i> = 0.000) and Tilt (ρ = –0.40, <i>p</i> = 0.009). The AVQI was lower (better) in samples that were evaluated as having a high amount of strain, but the difference was not significant. Only CPPS differentiated significantly between low and high amounts of creak. <b><i>Conclusion:</i></b> The AVQI does not seem to differentiate between high and low amounts of creak and strain in normophonic speakers.

Zusammenfassung Hintergrund Akustische Messverfahren haben das Potential Heiserkeit und Behauchtheit objektiv zu messen und der Acoustic Voice Quality Index (AVQI) und der Acoustic Breathiness Index (ABI) zeigen gute Ergebnisse hinsichtlich Validität und Reliabilität. Das Ziel dieser Studie war die Überprüfung der internen Validität der aktuellen Version des AVQI 03.01 und des ABI im Deutschen. Hierzu wurde die Sensitivität gegenüber Stimmveränderungen im Heiserkeits- und Behauchtheitsgrad durch logopädische Stimmtherapie untersucht. Material und Methoden Insgesamt wurden 84 Stimmaufzeichnungen von fortlaufender Sprache und dem Vokal [a:] vor und nach einer Stimmtherapie verwendet. Alle Aufnahmen der 42 Probanden stammten von Stimmpatienten mit unterschiedlichen Schweregraden einer Dysphonie vor und nach einer konservativen Therapie. Alle Stimmen wurden von drei Stimmexperten nach dem RBH-System beurteilt. Ergebnisse Die Intra- Beurteiler Reliabilität ergab insgesamt eine hohe Reliabilität für Heiserkeit (Kappa Mittelwert = 0,76) und Behauchtheit (Kappa Mittelwert = 0,69). Die Inter- Beurteiler Reliabilität erreichte hingegen eine niedrigere Reliabilität für beide Stimmqualitätsaspekte, die zwischen Kappa = 0,27 und 0,29 lag. Für beide Messverfahren, AVQI und ABI, zeigte sich ein deutlicher Zusammenhang zwischen der Perzeption von Veränderungen der Stimmqualität vor und nach der Therapie und dem Messergebnis (Heiserkeit vs. AVQI: r = 0,715, p < 0,01 und Behauchtheit vs. ABI: r = 0,712, p < 0,01). Die Unterschiede waren auch nicht signifikant. Schlussfolgerung Die Resultate zeigten, dass AVQI Version 03.01 und ABI eine hohe interne Validität haben, um Stimmveränderungen nach Intervention zu erfassen. AVQI und ABI sind zwei valide und robuste Messinstrumente, die Heiserkeit und Behauchtheit objektiv messen können.

Objective: The aim of this study was to validate the Acoustic Voice Quality Index 03.01 (AVQIv3) and the Acoustic Breathiness Index (ABI) in the Spanish language. Method: Concatenated voice samples of continuous speech (cs) and sustained vowel (sv) from 136 subjects with dysphonia and 47 vocally healthy subjects were perceptually judged for overall voice quality and breathiness severity. First, to reach a higher level of ecological validity, the proportions of cs and sv were equalized regarding the time length of 3 seconds sv part and voiced cs part, respectively. Second, concurrent validity and diagnostic accuracy were verified. Results: A moderate reliability of overall voice quality and breathiness severity from 5 experts was used. It was found that 33 syllables as standardization of the cs part, which represents 3 seconds of voiced cs, allows the equalization of both speech tasks. A strong correlation was revealed between AVQIv3 and overall voice quality and ABI and perceived breathiness severity. Additionally, the best diagnostic outcome was identified at a threshold of 2.28 and 3.40 for AVQIv3 and ABI, respectively. Conclusions: The AVQIv3 and ABI showed in the Spanish language valid and robust results to quantify abnormal voice qualities regarding overall voice quality and breathiness severity.
