Intertrochanteric Corrective Osteotomy as a Joint-Saving Surgical Treatment for Femoral Head Necrosis

Forty-seven intertrochanteric osteotomies were performed in 9 years on 40 patients with femoral head necrosis in Arlet and Ficat stages II and III in the Orthopaedic Department of the Medical School, Hannover. With an average follow up of 5.9 years (minimum 2.3 years, maximum 9.3 years), the study shows only 46% of the patients with satisfactory, good and very good results. After the joint saving operation, 26% of treated patients with pre-operative employment went into premature retirement. The average postoperative non-working period was 11 months (7-18 months). Clinical Relevance The results after the joint-saving operation were better when the operation was performed at an early stage of the disease with only a small area of necrosis. Although the operation will not prevent a progression of necrosis and a secondary arthrosis, we recommend that the joint-saving operation be performed during stages I and II to prolong the period before a total hip joint replacement becomes necessary. During stage III a total hip joint replacement may be preferable depending on clinical symptoms, patient age and the radiological findings (necrosis angle > 180°).

The sliding wear behavior of ultra high molecular weight polyethylene (UHMWPE) was examined on a novel low temperature degradation-free zirconia/alumina composite material and on the conventional ceramics (alumina and zirconia) used for a femoral head in total hip joint replacement. The wear of UHMWPE pins against these ceramic disks was evaluated by performing linear reciprocal sliding and repeat pass rotational sliding tests for one million cycles in a bovine serum. The weight loss of polyethylene against the novel low temperature degradation-free zirconia/alumina composite disks was much less than conventional ceramics for all tests. The mean weight loss of the polyethylene pins was more in the linear reciprocal sliding test than in the repeat pass rotational sliding test for all kinds of disk materials. Neither the coherent transfer film nor the surface damage was observed on the surface of the novel zirconia/alumina composite disks during the test. In conclusion, the novel zirconia/alumina composite leads the least wear of polyethylene among the tested ceramics and demonstrates the potential as the alternative materials for femoral head in total hip joint replacement.

Total Hip Joint Replacement Surgery is the procedure of surgical removal of cartilage in the hip joints damaged by disease osteoarthritis to replaced with artificial components. Total Hip Joint Replacement has been indicated to relieve pain in the pelvis due to degenerative joint disease. In Indonesia, this procedure has been practised, but due to the cost of expensive products resulting in limited only to upper classify patients. The high prices caused by importing prostheses products, the technological capabilities and the difficulty to meet the standards of medical devices. The regulation of the Minister of Health of the Republic of Indonesia Number 52, 2016 for standard rates in organising health services Health Assurance Program, explained that the action of these operations entered into the insurance claims BPJS program, but not for the prostheses. This paper discusses the introduction of the development of the hip joint prostheses made from a local material that’s SS 316L, PEEK and Ceramic Dental Stone. The result of this development is the product of the hip joint prostheses has a reasonable price with good quality so that it can become affordable for middle to lower patients.
