From domestication to imperial patronage: Deconstructing the biomedicalisation of occupational therapy

Occupational therapy knowledge emerged in the 19th century as reformist movements responded to the industrialisation of society and capitalist expansion. In the Global North, it was institutionalised by State apparatuses during the First and Second World Wars. Although biomedicine contributed to the rapid expansion and establishment of occupational therapy as a health discipline, its domestication by the biomedical model led to an overly regulated profession that betrays its reformist ideals. Drawing on the work of Deleuze and Guattari, our aim in this article is to deconstruct the biomedicalisation of occupational therapy and demonstrate how resistance to this process is critical for the future of this discipline. The use of arts and crafts in occupational therapy may be conceptualised as a ‘nomad science’ aesthetically resisting the domination of industrialism and medical reductionism. Through the war efforts, a coalition of progressive nurses, social workers, teachers, artisans and activists metamorphosed into occupational therapists. As it did with nursing, biomedicine proceeded to domesticate occupational therapy through a form of ‘imperial’ patronage subsequently embodied in the evidence-based movement. ‘Occupational’ jargon is widely used today and may be viewed as the product of a profession trying to establish itself as an autonomous discipline that imposes its own regime of truth. Given the symbolic violence underlying this patronage, the future of occupational therapy should not mean behaving according to biomedicine’s terms. As a discipline, occupational therapy must resist the appropriation of its ‘war machine’ and craft its own terms through the release of new creative energy.

When we think of the Victorian era, images of shrouded piano legs, dismal factories and smoggy streets often come to mind. However, the 19th century has been rediscovered in recent years as the home of something quite different: bold utopian visions of the future. William Morris’ great literary utopia News from Nowhere, first published in 1890, is an interesting case study in this context. Morris’ text is the point of departure for a number of recent returns to Victorian utopianism, including Sarah Woods’ updated radio adaptation of News from Nowhere (2016) and the BBC’s historical reality television series The Victorian House of Arts and Crafts (2019). In this article, I analyse these Morris-inspired texts with the aim of exploring the place of old visions of the future in the contemporary cultural imaginary. Building on previous work in neo-Victorian studies and utopian studies, the claim is made that the return to 19th-century dreams is a plural phenomenon that has a number of divergent effects. More specifically, neo-Victorian utopianism can function to demonstrate the obsolescence of old visions of utopia, prompt a longing for the clarity and radicality of the utopias of the Victorian moment, or encourage a process of rejuvenating the utopian impulse in the present via a detour through the past.

Present concerns about the state of theory and practice in occupational therapy can be explained in light of past developments, both successful and unsuccessful efforts. This article explains why occupational therapists still use arts-and-crafts modalities, the practice of which can be traced to early therapists influenced by the philosophy and practices of a social movement that swept across America from 1895 to 1920. This analysis is based on historical research, a methodology not common in occupational therapy literature.

Background. While occupational therapy’s inception was from the Arts and Crafts movement and the moral treatment movement with war veterans, the profession has evolved to requiring a professional entry-level master’s degree to practice, and involves complex relationships with clients across the life span. Throughout history, a consistent impact of each industrial revolution has been the loss of jobs to automation. This consequence is even more profound today with the exponential growth of innovations and automation. Purpose. The objectives of this article are to (a) set the context by reviewing the evolution, or five eras, of occupational therapy in Canada; (b) present what is meant by the “Fourth Industrial Revolution”; and (c) examine the technological innovations faced by occupational therapists and our clients as we enter the “sixth” era of occupational therapy in Canada. Key Issues. Although occupational therapy, as a profession, has low risk for automation, a great number of our clients will not be able to reskill fast enough to keep up with job market requirements. Telerehabilitation, the Internet of Things, virtual reality, 3-D printing, robotics, artificial intelligence, and autonomous vehicles are challenging ways occupational therapists provide services to clients. Implications. It is recommended that occupational therapists engage with disciplines beyond current typical connections, as our expertise is called upon to advocate for ourselves and our clients who are end users of these technologies.
