The authors report a case of acute vagus nerve paralysis that appeared during a course of chemotherapy. The drugs had been administered through a totally implantable venous access device (TIVAD), whose catheter tip had migrated into the right internal jugular vein (IJV) and was surrounded by a complete venous thrombosis. The supposed aetiology of this paralysis was a leakage of the cytotoxic drug (5-fluorouracil) from the vessel wall into the surrounding carotid space, because of the stagnation of the chemotherapeutic agent above the thrombosis. Four months after cessation of chemotherapy, the laryngeal paralysis was still evident.

It is important to correctly distinguish paragangliomas from other tumors such as schwannomas in the preoperative assessment of head and neck tumors because paragangliomas have a propensity to bleed profusely during surgery. Therefore, preoperative embolization is often required while with schwannomas preoperative embolization is generally not required. Occasionally, schwannomas can mimic paragangliomas on routine computed tomography and magnetic resonance imaging of the neck. In this study, we retrospectively evaluated the computed tomography angiography of the neck of 10 patients with carotid space tumors. Seven patients had pathologically proven paraganglioma while three patients had schwannomas. We describe the “computed tomography angiography lightbulb sign” as avid homogeneous enhancement in the arterial phase which can accurately distinguish these entities.

This case report describes a case of carotid space abscess secondary to lateral sinus thrombosis associated with internal jugular vein thrombosis. With this case, we illustrate a rare entity that presented in an extremely rare manner. To the authors knowledge such a case has not been previously reported.
