The implementations of RI-ISI can improve the substantial cost reductions as well as dose reduction. From the points of the safety concern, the achievements of these procedures can identify the inspection rank to promote the integrity of the present inspection program. Thus, this paper presents the applications of RI-ISI to recirculation piping welds sensitized to IGSCC of Taiwan BWR-4 nuclear power plant. The establishment of the risk evaluation can be referenced for the regulatory and plant maintenance.

The term Errors of Commission (EOC) describes a safety problem of complex technical systems where humans intervene into the system functions in an inappropriate way. Most serious events in any technical area are caused by EOCs. Humans shut down a high pressure injection that is needed in the current situation (TMI Nuclear Power Plant), humans decide to continue a test that brings the plant into serious conditions (Chernobyl Nuclear Power Plant), Maintenance Management does not exchange a wheel that is far below the safety margins (Eschede Railway Accident in Germany). Similar examples can be mentioned in other technical systems like Aircraft or Medicine for instance. This paper firstly gives an overview about the problem and the work in that field. It then focuses on some approaches for solution concerning the modeling of cognitive human behaviour.

In this paper, an algorithm concerning the primitive action affordances of learning nuclear power plant maintenance robot is presented. The algorithm generates a random matching data set through a new matching method, which is utilized for the selection of object operation, with the matching rate improved by trial and error, and then the attempt number for a successful operation is reduced. In the end, simulation is conducted to verify the feasibility and correctness of the proposed algorithm.
