McWilliams and Dunn report a case of aplastic anemia in Down's syndrome similar to our recently reported case. We can expect a flurry of other such case reports. However, since the association of aplastic anemia and Down's syndrome is still unusual, it will be interesting to document difference in predisposing factors and response to various forms of therapy.

The first case report of aplastic anemia in Down's syndrome (Weinblatt ME, Higgins G, Ortega JA: Aplastic anemia in Down's syndrome. Pediatrics 67:896, 1981) may stir up a flurry of other such case reports. We are studying a 27-month-old black girl with a 47,XX + 21 karyotype who had been doing well following repair of an endocardial cushion defect at age 10 months, until she was seen with petechiae and purpura at age 19 months.

AbstractDown's syndrome has its own dysmorphic findings and is accompanied by mental retardation and hypotonia. Klinefelter's syndrome is a syndrome caused by a numerical abnormality that affects male physical and cognitive development. This case reports a unique finding of 48,XXY, + 21 and a current literature review. A 4-month-old male patient presented with typical clinical features of Down's syndrome with hypothyroidism, atrial septal defect, ventricular septal defect, and patent ductus arteriosus without any phenotypic signs of Klinefelter's syndrome.

Until recently, it was considered that Down's syndrome precluded a diagnosis of mania, or gave rise to an atypical presentation. There have been seven case reports of mania in people with Down's syndrome and all these cases are reviewed. The clinical features of mania are noted to be similar to those previously described in individuals with learning disabilities due to other causes. However, all reported cases are male and none has a family history of affective disorder. In two of the seven men reported, the illness followed a rapid cycling pattern. Hypothyroidism and monoamine biochemistry in people with Down's syndrome are discussed in the context of these atypical features.

A child with Down's syndrome was seen with pancytopenia and was found to have an aplastic bone marrow. None of the known contributing etiologies of aplastic anemia could be identified in this patient. This is the first reported case of the simultaneous appearance of these two disorders.
