Bucco-Parapharyngeal Occurrence of Pleomorphic Spindle Cell Lipoma-A Unique Entity: Case Report

Lipomas are common benign neoplasms of oral region which occur scarcely in the parapharyngeal space (PPS). The diversity observed in its histologic presentation classified it into many subtypes among which Spindle Cell Lipoma (SCL) is a rare entity. SCL is an uncommon benign tumor of adipose tissue usually located superficially in the neck, back and shoulder region but its presentation in oral and pharyngeal spaces is very rare. Exhaustive literature search for oral and pharyngeal space pleomorphic SCL till date revealed 45 cases of oral SCL with a mean age distribution of 55 years. Only one reported case of parapharyngeal occurrence of pleomorphic SCL was found. A rare case of pleomorphic variant of SCL in PPS extending into the buccal space is being presented. This case could be the first to report the multiple presentation and the youngest age of occurrence of SCL in PPS.

ABSTRACT Spindle Cell Lipoma (SCL) is an uncommon benign tumor of adipose tissue that is usually super􀃶cially located in the posterior neck, back and shoulder region. We report a rare case of SCL in a 39 years old male presenting as neck swelling with c/o stridor and dysphagia. The contrast enhanced computed tomographic scan of neck showed an external and internal part of the swelling traversing through the thyrohyoid membrane suggestive of Mixed Pyolaryngocele. The tumor was excised by a trans-cervical approach but the post operative histopathology was found to be spindle cell Lipoma. We report this case due to its atypical presentation and location. Spindle cell lipomas are usually located in the subcutaneous tissue of the back, shoulders, and neck. To our knowledge, the presence of such a tumor in the parapharyngeal space has not yet been described. We evaluated a 45-year-old man with a tender swelling of the right parotid area that had reached the submandibular area. Clinical examination and magnetic resonance imaging revealed the presence of a tumor that coated the parotid area laterally and extended into the center of the parapharyngeal space, thus causing a dislocation of the pharyngeal muscles and mucosa. We performed a total parotidectomy and submandibulectomy on the right side and extirpated the parapharyngeal tumor. We were able to spare the facial nerve, and no facial paralysis occurred. Histologic examination revealed an atypical lipomatous tumor with a remarkably large portion of spindles.

Spindle cell lipoma of the hypopharynx is an extremely rare entity. Here, we present the first case of this lesion originated in the cricopharyngeal region, with symptoms of chronic progressive dysphagia, which can be confused with other pathologies; endoscopic and magnetic resonance imaging (MRI) evaluation are the methods of choice for its diagnostic approach. The best therapeutic approach is endoscopic resection with rapid recovery and few complications. Long-term followup is recommended, either endoscopic or imaging, given that it can be confused with an undiagnosed liposarcoma; additionally, its long-term behavior is unknown.

Odontogenic myxofibroma is a rare, benign tumor, which is considered to be the variant of odontogenic myxoma. It is locally infiltrative, aggressive and has high recurrence rate. Only 24 specific cases of myxofibroma have been reported since 1950 and only two cases have been reported with excessive calcifications till 2012. Here, we report a rare case of Odontogenic myxofibroma with calcifications in 53 years old female patient who presented with the chief complain of swelling. The final diagnosis of Odontogenic myxofibroma was established after histopathological and immunohistochemical examination.

We report a rare case of a parotid oncocytoma in a 63-year-old woman. It is a rare benign tumor accounting for less than 1.5% of all salivary gland tumors. It is known as the parotid mitochondrioma because it is made of cells rich in mitochondria with granular cytoplasm. It is a rare entity poorly documented in the literature, and there are no pathognomonicsymptoms or imaging, which makes the diagnosis of this tumor challenging.Surgery is the main treatment, and the diagnosis is confirmed by the histological study of the specimen. Through this observation, and in light of the literature, we will underline the anatomoclinical and radiological peculiarities of this tumor, to induce clinicians to consider this histological type.

Abstract Objective Rare case of intracapsular carcinoma ex pleomorphic adenoma in the buccal space with focus on its appropriate management. Method Case report with review of current literature on buccal space lesions and intracapsular carcinoma ex pleomorphic adenoma. Result Although salivary gland tumors are common, intracapsular carcinoma ex pleomorphic adenoma is a rare entity especially within the buccal space with no similar case report in the current literature. Conclusion Our case is unusual due to its unusual presentation in the buccal space and pathological features. This case report also highlights issues regarding its appropriate management.
