A rare case of a stab wound.The case described by the author is interesting for some complications, which accompanied the injury, and for the excellent outcome, despite the severity of the injury and complications. The patient received a wound corresponding to the 7th rib on the anterior axillary line. When the 7th rib and 6th intercostal space were exposed, it appeared to be torn apart, a cloudy liquid with a sour smell protrudes from the wound (when exhaled); Listening to the lung reveals the absence of respiratory noises. In the circumference of the wound, subcutaneous emphysema.

Objective To compare the anterior with lateral mean chest wall thickness measured by bedside ultrasound in Chinese adults in order to suggest a preferred site for needle decompression in tension pneumothorax. Study design This was an observational cross-sectional study conducted in a regional hospital over three months. Subjects were recruited by convenience sampling. Chest wall thickness at the second intercostal space, mid clavicular line, fifth intercostal space, anterior axillary line and fifth intercostal space and mid axillary line was measured using ultrasound on both sides. Range, mean values and confidence intervals were calculated. Results One-hundred and fourteen subjects were recruited. The mean anterior chest wall thickness was 2.62 cm (at second intercostal space, mid-clavicular line) and mean lateral chest wall was 2.68 cm (at fifth intercostal space, anterior axillary line) and 2.87 cm (at fifth intercostal space and mid-axillary line) respectively. Chest wall thickness at fifth intercostal space and mid-axillary line was significantly greater than second intercostal space, mid-clavicular line ( p < 0.01). Chest wall thickness was greater than 5 cm in 3.5% of the study population. Conclusion There is a need for population-based guidelines. We recommend needle decompression at the second intercostal space, mid-clavicular line with a 50-mm angiocath for Chinese patients with tension pneumothorax. A lateral approach at the fifth intercostal space, anterior axillary line may be considered as an alternative in case of failure. Prehospital point-of-care ultrasound may be a useful adjunct in managing such patients.

Pneumomediastinum, and subcutaneous emphysema usually result from spontaneous alveolar wall rupture and, far less commonly, from disruption of the upper airways or gastrointestinal tract. Subcutaneous neck emphysema, pneumomediastinum, and retropneumoperitoneum caused by nontraumatic perforations of the colon have been infrequently reported. The main symptoms of spontaneous subcutaneous emphysema are swelling and crepitus over the involved site; further clinical findings in case of subcutaneous cervical and mediastinal emphysema can be neck and chest pain and dyspnea. Radiological imaging plays an important role to achieve the correct diagnosis and extension of the disease. We present a quite rare case of spontaneous subcutaneous cervical emphysema, pneumomediastinum, and pneumoretroperitoneum due to perforation of an occult sigmoid diverticulum. Abdomen ultrasound, chest X-rays, and computer tomography (CT) were performed to evaluate the free gas extension and to identify potential sources of extravasating gas. Radiological diagnosis was confirmed by the subsequent surgical exploration.

A rare case of painful snapping around the Achilles tendon without reported injury in a healthy 30-year-old male amateur runner is reported. The plantaris tendon was firmly attached to the Achilles tendon by adhesions, and its movement was restricted and impaired. All adhesions were removed and the plantaris tendon was liberated during surgery, with an excellent outcome. This case showed that previously described removal of the snapping tendon is not necessary because liberation of the plantaris tendon restored function of both the plantaris and the Achilles tendons.

Objective. To present a rare case of extensive subcutaneous emphysema and spontaneous pneumomediastinum following ingestion of Ecstasy in a young adult. We also review the relevant literature and discuss how this case supplements it.Case Report. We report a case of a 19-year-old man with a history of painless neck and chest swelling, and no chest pain or breathlessness, after consuming Ecstasy tablets. Radiological imaging showed evidence of pneumomediastinum and extensive subcutaneous emphysema. The patient remained well under observation and his symptoms improved with conservative management.Conclusions. Subcutaneous emphysema and pneumomediastinum after Ecstasy ingestion is uncommon. Cases are often referred to the otolaryngologist as they can present with neck and throat symptoms. Our case showed that the severity of symptoms may not correlate with severity of the anatomical abnormality and that pneumomediastinum should be suspected in Ecstasy users who present with neck swelling despite the absence of chest symptoms. Although all cases reported so far resolved with conservative management, it is important to perform simple investigations to exclude coexisting serious pathology.

<p class="abstract">Floating knee injuries are commonly seen nowadays due to increase in road traffic accidents. However, bilateral presentation of floating knee is a rare injury and challenging to orthopaedicians for a good functional outcome. We hereby present a rare case of bilateral floating knee injury in a 45 year old male treated with open reduction and internal fixation in a staged procedure. Functional assessment was done using modified Karlstrom-Olerud criteria which showed excellent outcome. Union was achieved in approximately 7 months. Decreased knee movements over left side as compared to right side was only complication seen.</p>

Suicide, or the deliberate act of killing oneself, accounts for a substantial number of unnatural deaths each year in many countries. Though death by self-stabbing is quite uncommon, the left part of the chest represents one of the elected sites of self-inflicted wounds because of the knowledge of the situation of the heart and that the injuries to the heart are severe. Subcutaneous emphysema is rare during or after dental procedures (usually extractions). Here, we describe the case of a 65-year-old woman who developed massive cervicothoracic subcutaneous emphysema and pneumomediastinum during a dental hygiene procedure employing an artificial airflow. She was diagnosed based on clinical manifestations and computed tomography (CT). CT revealed massive subcutaneous emphysema extending from the superior left eyelid to the diaphragm. We describe the clinical and radiological characteristics of this rare case.

AbstractBilateral vertex extra dural hematomas (EDH) are a rare form of extra dural hematomas with many unique features. We report one such case of bilateral vertex extra dural hematoma. A young house wife had a fall in the bathroom and sustained head injury. She showed clinical features of progressive raised intra cranial pressure. Neuroimaging revealed bilateral vertex EDH. The larger left sided hematoma was evacuated surgically while the smaller right sided hematoma was managed non operatively, with excellent outcome. This case report presents this rare case and briefly reviews the literature. Vertex EDH has a special uniqueness as to their etiopathology, clinical presentation, diagnosis & management principles.
