Cutaneous leishmaniasis is an endemic diease in the middle and south of Iraq. distribution of infection was studied in two governorates (diwanya and hilla), total number of patients attended to the dermatology center in three months (December 2016, January, February 2017) in diwanyia was 1940, 132 were cutaneous leihmaniasis (10.7%) while in hilla 1757 patients were attended to the dermatology center at the asme months, cutaneous leihmaniasis cases were 42 (2.4%). total number of cutaneous leishmaniasis was 208, 132 for diwanya 76% while for hilla was 42, 24%.infected males were higer than females in both cities 74.3%,25.7% respectibely. Age variations reveal all infected cases were in all ages but high percent were in age of ten years old and below 35.6%. no cases were recorded during summer months and the peak of this sisease was in January 28.5%, February 30.9%. pathological lesions were classified grossly as follows, papule (redness and swelling ) pustule (redness with crusts sometimes with pus)ulcer (it was no crusts with shallow ulceration irregular margin)scar formation. Hematological study revealed increase in number of erythrocytes and variation in size (poikilocytosis). Treatment were mostly, systemic injection with topical treatment, several types of ointment.

ABSTRACT Current treatment of cutaneous leishmaniasis (CL) relies mainly on pentavalent antimonials salts and second-line drugs include pentamidine and amphotericin B, but these therapies have side effects and require parenteral administration. The aim of this work was to evaluate the topical formulations containing pentamidine isethionate (PI) in the experimental treatment of cutaneous leishmaniasis (CL). Golden hamsters (Mesocricetus auratus) were infected in the nose with Leishmania (Leishmania) amazonensis. Six treatment groups received different topical treatments of anhydrous or hydrating emulsions, for a maximum of 10 days, with an application of 50 mg day-1. After treatment tissue samples of lesions were evaluated by histology, transmission electron microscopy and biopsy cultivation. Compared with untreated group, topical treatment with hydrating emulsion with 10% PI and usnic acid (ACE5AU) showed significantly decrease in volume lesion (P= 0.028) on 20th day after the end of the treatment with reduction of 27.37%. Topical treatment with anhydrous emulsion with 10% PI and usnic acid (ACPU) reduces parasite burden in Golden hamsters. This study demonstrated the potential of topical treatment to reduce the number of parasites that could be combined with others drugs and to have a faster and more effective treatment of cutaneous leishmaniasis.

Introduction Traditional Chinese Medicine (TCM) use has been believed to be common in Hong Kong but no data existed on its prevalence among emergency patients. We conducted a prospective survey to study the prevalence of its use, the type of TCM use, frequency and nature of associated adverse reactions. Methods All accident & emergency department (AED) attendances at the United Christian Hospital were screened by the triage nurse and attending doctor for TCM use and its type in a consecutive 31 day period. The causal relationships between the TCM and their presenting illness were also assessed and decided by the doctor in-charge. Results 291 out of 21,475 patients (1.4%) reported TCM use within one week. Among them, 117 (40.2%) consumed Chinese herbal medicine, 75 (25.8%) took proprietary Chinese medicine, 1 (0.3%) took both, 96 (33.0%) received topical treatment and 2 (0.7%) were unclassified, with 22 out of the 291 TCM users (7.6%) presented for TCM-related toxicity. Most complications were mild, the commonest ones being dermatitis after topical treatment (68.2%) and allergic reaction after oral intake (22.7%). Conclusion A small number of patients took TCM treatment shortly before AED consultation. Most TCM-related toxicities were mild and the commonest reactions were dermatitis and systemic allergy.
