The study objectives were to investigate the prevalence and to determine the most frequent causes of coma in patients in Ryazan; to determine the frequency of fatal outcomes at the stage of medical evacuation and the frequency of hospitalizations; to identify the features of emergency medical care in the prehospital period. Materials and methods. We analyzed statistical data on the number of witnesses who applied for emergency medical aid in cases of disturbance of consciousness in patients who were subsequently diagnosed as comatose patients; we revealed main causes and types of comatose states, number of fatal outcomes and frequency of hospitalizations in Ryazan. Materials of the study – cards of calls of ambulance crews in Ryazan in 2016-2020. Results of the study and their analysis. Analysis of statistical data for Ryazan in 2016-2020 showed a steady increase in the number of ambulance calls to patients in coma. In Ryazan, as in Russia as a whole, cerebral coma prevailed, with diabetic coma in second place and toxic coma – in third. In 2016-2020, the proportion of coma-related deaths was 2.7% in the prehospital period and had no upward trend. In 84% of cases patients were hospitalized in medical treatment organisations, patients with hypoglycemic coma sometimes refused hospitalization, there was no tendency in increase of the number of refusals. In the prehospital period, ambulance care for patients with coma was provided in accordance with the algorithms, average time to reach the call was 12.4 min, which corresponds to the respective norms for emergency calls for Ryazan.

The aim of the study is to study the prevalence, structure and nature of injuries, to identify the frequency of deaths at the stage of medical evacuation, and to determine the number of injured people who need medical care in medical organizations (LMO), according to the data on requests of the population of the Ryazan region and the city of Ryazan for emergency medical care (SMP). Materials and methods of research. Statistical data on the requests of victims with injuries for emergency medical care in the Ryazan region and the city of Ryazan are analyzed. Materials of the study – maps of calls of the SMP teams in Ryazan and statistical data for the Ryazan region for 2017-2019. The results of the study and their analysis. The most frequent causes of injuries, their nature, the frequency of requests of the population of the Ryazan region and the city of Ryazan for emergency medical care, the number of deaths and the frequency of hospitalizations for injuries were determined.

Aim. To study the structure of appealability of the population for emergency medical care for cardiac reasons, and the conformity between the cardiac cause and diagnosis. Methods. Analyzed was the statistical data of appealability of the population for emergency medical care for cardiac reasons in a large industrial center of the Republic of Tatarstan, city of Kazan. Register cards of ambulance calls for the year 2010 served as materials for the study. Results. It was established that cardiac reasons account for 30% of all adult requests for emergency medical care. The main flow (96.4%) was formed by the primary appeals to the ambulance station, and much less frequently (3.6%) by primarily detectable visits to health institutions for acute forms of ischemic heart disease and arrhythmias. Analyzing the structure of cardiac reasons in nature of the requests for care it was revealed that the proportion of disease with a sudden cardiac profile is 37.1%, with acute care of different profiles - 44.5%, sudden illness of other profiles - 16.5%, other - 1.9%. The proportion of disease with a sudden cardiac profile in the structure of other reasons for seaking medical care is 5.5%. During primary referral to the ambulance station the frequency of diseases with a sudden cardiac profile varies depending on the reason and ranges from 24 to 38.9%. Conclusion. These established features of the analysis of appealability of the population for emergent medical care for cardiac reasons provides the basis for the establishment and improvement of the algorithm of questions to ask the caller of an ambulance and its introduction into the work of the dispatching service, which will increase the frequency of conformity of the cause and diagnosis.

Relevance. The probability of emergency situations in the Barents region determines the need of improvement of forces and means of rendering first and emergency medical care to victims. In September 2017 the large-scale international exercise of rescue services of Russia, Norway, Sweden, and Finland under the code name “Barents-Rescue 2017” was carried out in the Republic of Karelia. The exercise worked oved rendering first-aid and emergency medical care to victims of emergency situations.Intention. To summarize experience in preparation, rendering and result assessment of first-aid and emergency medical care to victims of emergency situations in the framework of the international exercises.Methods. Medical document analysis (hospital records, medical triage records, expert assessment records), statistical analysis of groups using Student’s t-test. The exercise involved 76 victims with injuries of various severity levels, among them 30 were of minor severity, 19 were of medium severity and 27 were of heavy severity. They received first-aid and emergency medical care by 42 rescuers and 67 health workers (doctors, paramedics, nurses) of rescue services of Russia, Norway, Sweden, and Finland. Results assessment was made on the basis of the developed hospital records, medical triage records and expert assessment records and also statistical analysis methods.Result and their analysis. The average expert assessment of a first-aid rendered by rescuers was sufficiently high and was equal to 4.6 ± 0.1 on a 5-score rating system, where 5 points means providing full assistance in accordance with the established standards of its provision. A qualitative medical triage was provided competently, the ordering of evacuation was right, life-threatening damages and injuries were properly defined. At the stage of medical evacuation all the victims were provided with full assistance, there were no divergence of diagnoses, some differences from final diagnose were permissible at the stage of medical evacuation and had no effect on victim’s condition. In addition, a part of the victims in the hospital had medical cards, where was information about the diagnoses and the measures taken. These medical documents were also analyzed in terms of the timeliness and quality of the provision of specialized medical care to injured people in the hospital.Conclusion. The results of expert assessment during the international exercise of rescue services of Barents Rescue 2017 showed that the quality of first-aid, medical triage and emergency medical care during the evacuation and also in the hospital is high enough, but there were some remarks too. Suggestions and comments were addressed to the Exercise organizing committee to improve the engagement during first-aid and medical care to victim of the emergency and they were included in the final report on the exercise.

The activity is analyzed of specialists of the Scientific and practical centre for emergency medical care (CEMP) of Department of health of the city of Moscow (DZM) in the context of the COVID-19 pandemic. The structure of trips of CEMP teams in the fight against COVID-19 from March 1 to July 1, 2020 is presented. It is noted that although all the structures under the jurisdiction of the DZM, starting from March 2020, are working on high alert in connection with the threat of the spread of coronavirus, CEMP specialists continue to work on providing emergency medical care (EMP) and medical evacuation of victims of road accidents and other emergencies, as well as emergency somatic patients with various nosologies.

Relevance. Acute respiratory failure (ARF) and acute cardiopulmonary failure (ACPF) are unavoidable, and often – the leading syndromes in any critical condition, especially in case of multiple organ failure syndrome as a regular stage in the course of previously incurable conditions. Even a full range of intensive care measures in this category of patients does not always give the desired result. According to various authors, with the development of acute respiratory distress syndrome, the mortality rate reaches 80 %.Intention is to determine the role of the aviation medical teams of the emergency medical service-disaster medicine in ensuring the timely availability of high-tech methods for patients who need them, regardless of the patient’s location.Methodology. Activities of the aviation medical teams of the Scientific and Practical Center for Emergency Medical Care of the Moscow City Health Department for the medical evacuation of patients and victims in emergency situations with ARF and ACPF were analyzed.Results and Discussion. There were analyzed interactions of aviation medical teams with the emergency medical response units of the territorial center for disaster medicine and with medical organizations during the medical evacuation of patients with ARF and ACPF. An algorithm and a scheme of interaction between air medical teams are proposed to optimize medical evacuation of these patients.Conclusion. An analysis of the activities of air medical teams revealed their advantages in providing care for patients with ARF and ACPF.
