Tuberculoma is not an uncommon lesion in intracranial space specially in developing world but tuberculoma in the cavernous sinus is very rare, and only less than ten cases have been reported in the literature, till today. Preoperative neuro-radiological features of such lesions may mimic neoplastic lesions of skull base and brain and post operative histopathological study brings the ultimate diagnosis. Here we report a rare case of cavernous sinus tuberculoma where tuberculomas were also in temporal lobe and Basal subarachnoid spaces (Right cavernous sinus, left temporal lobe, right sylvian fissure, basal cistern, interpeduncular cistern and prepontine cistern).   DOI: http://dx.doi.org/10.3329/jom.v13i1.10056  JOM 2012; 13(1): 75-76   Tuberculoma is not an uncommon lesion in intracranial space specially in developing world but tuberculoma in the cavernous sinus is very rare, and only less than ten cases have been reported in the literature, till today. Angioleiomyomas are benign solitary soft-tissue tumors that originate from the vascular smooth muscle of vessels and rarely occur in the hand. Pain and tenderness are the only clinical characteristics. This report presents a rare case of angioleiomyoma arising from both the superficial palmar arch and a concomitant vein of the artery. The MRI findings lack specific radiological features. However, three-dimensional magnetic resonance angiography demonstrated the existence of a relationship between the tumor and its feeding vessels. The diagnosis was made after a histopathological study of the tumor. Adequate collateral circulation should be confirmed both before and during surgery if the lesion occurs within an artery. Angioleiomyoma should therefore be included in the differential diagnosis of painful vascular tumors with no neurological findings in the hand.

Herein, we report on a rare case of craniopharyngioma arising in the left temporal lobe with no prior history of head trauma or surgery. There was a solid-cystic mass in the left temporal lobe on MR images. To the best of our knowledge, this is the second case of a craniopharyngioma occurring in the temporal lobe.

Tuberculosis involving skull base and brain is relatively common in developing countries. Preoperative neuro-radiological features of such lesions mimic neoplastic lesions of brain & skull base and postoperative histopathological study brings the ultimate diagnosis. Here we present a case of large tubercular lesion involving retrobulbar, infratemporal and middle cranial fossa that preoperatively thought to be a malignant lesion in a young child and was managed surgically with anti-tubercular drugs having a happy termination. Even radiologically malignant looking mass lesion in brain and skull base can be proved as tuberculosis that can bring happiness to the patient and also to the treating neurosurgeons. Epilepsy is a chronic neurological disorder that is characterized by episodes of seizure. Sexual dysfunction has been reported in patients with seizure, which mostly manifests as erectile dysfunction and premature ejaculation in men. In this study, we report the case of a 65-year-old Chinese man with frequent spermatorrhea. Electroencephalography suggested local epilepsy in the left temporal lobe. After treatment with anti-epilepsy drugs, the symptoms disappeared and did not recur. To the best of our knowledge, this is the first reported case of epilepsy-induced spermatorrhea. The symptoms of spermatorrhea are probably a rare manifestation of seizure. When repetitive stereotyped symptoms occur, seizure should be considered, and tentative anti-epileptic treatment may be a good option.
