The Kyrgyz Republic holds one of the first positions in the world in terms of mortality and morbidity of brain stroke. Open sources provide data from the Kyrgyz Republic on the analysis of morbidity and mortality, and medication treatment of patients with various forms of brain stroke, but there are almost no studies on primary disablement and subsequent rehabilitation measures, which are based on setting a rehabilitation diagnosis and defining the goals of rehabilitation. The purpose of this study is to analyze problems in making a rehabilitation diagnosis for persons with disabilities with the consequences of brain stroke, based on the examination of the records of 3 medical and social expert commissions (MSEC) of the Kyrgyz Republic, out of 27 existing commissions. The authors present the results of a 2018–2019 study of the medical and rehabilitation documents of 328 persons with primary disablement in order to determine the rehabilitation diagnosis in these documents as a basic tool for the formation of a clinical rehabilitation route. Despite the availability of regulatory documents, clinical guidelines on the management of stroke patients, there is no evidence of a rehabilitation diagnosis in the documents reviewed, which affects the effectiveness of rehabilitation of persons with disabilities with the consequences of brain strokes. The authors recommend a set of measures to eliminate the barrier revealed and to develop rehabilitation services. Such rehabilitation services could be triggered by introducing the International Classification of Functioning, Disability and Health into the field of rehabilitation.

The Second National Conference on Disability, held in Bari in 2003, took the World Health Organization's International Classification of Functioning, Disability and Health (ICF), adopted by all WHO member states in 2001, as its frame of reference for future action and policies on disability. The ICF broke decisively with the medical model by seeing disability as an interaction between a biological and psychological condition and environmental and attitudinal barriers. Although existing Italian legislation on access to work for persons with disabilities, particularly Law 68/1999 on ‘collocamento mirato’ (targeted placement), anticipated some of the principles and definitions of the ICF, its implementation in practice was often snared in complex bureaucratic procedures and compromised by narrowly medical assessments of impairment and by considerable variations in standard from region to region. In 2009–2011 a pilot project, Progetto ICF4, was launched in 11 regions of Italy. It applied ICF principles, using Social Network Analysis (SNA) to assess the suitability of a work environment in terms of the networks of relations between the different actors involved in it. The way this has functioned in practice is illustrated by a case study of Teramo, one of the provinces in the pilot.

Tendo em vista a importância do caráter interdisciplinar dos serviços de reabilitação de pessoas com deficiência, buscou-se apresentar a avaliação interdisciplinar utilizada em um Centro Especializado em Reabilitação do Recife. A avaliação, baseada na Classificação Internacional de Funcionalidade, Incapacidade e Saúde, faz parte do processo de admissão dos pacientes ao serviço de reabilitação e conta com a participação de Terapeuta Ocupacional, Fisioterapeuta e Fonoaudiólogo, com o objetivo de atender as diferentes demandas dos pacientes com comprometimento neurológico de maneira cooperativa entre as profissões Logo, foi possível perceber sua eficiência na visualização do sujeito integral, permitindo maior precisão no planejamento terapêutico. Abstract: Bearing in mind the importance of the interdisciplinary character of rehabilitation services, this analysis seeks to present an interdisciplinary evaluation used in a specialized rehabilitation center in Recife. The evaluation, based on the International Classification of Functioning, Disability and Health, is part of the patients admission process. Occupational therapists, physiotherapists and speech therapists take part in the process. It aims to meet different demands from neurologically demaged patients in a cooperative manner among the three perspectives. Therefore, it was possible to notice its efficiency in seeing the patient as a whole, bringing greater precision to the therapeutical planning.Key words: Rehabilitation; International Classification of Functioning, Disability and Health; Disability Evaluation.Resumen: Dada la importancia del carácter interdisciplinario de los servicios de rehabilitación de personas con discapacidad, se buscó presentar la evaluación interdisciplinaria de un Centro Especializado en Rehabilitación del Recife. The article describes modern approaches to the organization of rehabilitation in patients with limb amputations and exarticulations: it highlights technology of using the International Classification of Functioning, Disability and Health, as a tool allowing to form rehabilitation diagnosis, rehabilitation prognosis, assess rehabilitation potential, determine the purpose and plan of rehabilitation, as such as to give recommendations at end of rehabilitation course in the process of rehabilitation in patients with amputations. Taking in to account the limitations of mobility and self-service opportunities assessed by the rehabilitation routing scale, we propose a tactics for patients’ transfer from the first to subsequent rehabilitation stages. The presented approach allows to optimize the rehabilitation process and to achieve designed results.

Ratification of the Convention on the Rights of Persons with Disabilities in Kyrgyzstan in March 2019 has encouraged the country to explore the possibilities of multidisciplinary tools to assess the needs and strengths of an individual. The International Classification of Functioning, Disability and Health (ICF) is such a recognized tool. ICF allows to have complete information on any individual using a system for coding the level of disability. Further, this assessment of an individual's needs and strengths becomes the basis for rehabilitation programs for individuals with disabilities. The purpose of this study is to examine the relationship between the profile of individual functioning compiled according to ICF and medical rehabilitation programs for brain strokes using the 2020 data of certain medical and social expert commissions (MSEC) of the Kyrgyz Republic as an example, followed by the preparation of recommendations for the development of medical rehabilitation system for brain strokes. Each of the selected individual with disability was given an adapted profile of individual functioning with domains corresponding to brain strokes. Analysis of the profiles showed the possibility of using a single tool in the usual practice of the health system and medical and social examination for a more detailed assessment of functioning impairments and environmental conditions of an individual, which allowed to re-orient the rehabilitation goals. Authors revealed insufficient rehabilitation resources in Kyrgyzstan, including the availability of medical personnel and medicines with evidence-based effectiveness. Analysis of the findings allowed the authors to present recommendations for the development of a medical rehabilitation system for brain stroke in the Kyrgyz Republic.
