Background: The present study examined the relationships among macular microvasculature, retinal structure, and epiretinal membrane (ERM) and explored the utility of optical coherence tomography (OCT) angiography (OCTA) in idiopathic ERM assessment. Methods: The study sample comprised 276 eyes of 276 patients. A total of 154 eyes with ERM and 122 normal (control) eyes were analyzed. Only one eye of each participant was randomly selected for posterior segment imaging. Each patient underwent OCT and OCTA. Images were analyzed with AngioTool 0.6. Results: Foveal avascular zone was significantly smaller in the ERM group (p = 0.044). Average retinal thickness and foveal thickness were significantly higher in the ERM group (both p = 0.001). Moreover, 64 (41.5%) patients exhibited no metamorphopsia, while 46 (29.8%) and 44 (28.7%) patients exhibited moderate and extensive metamorphopsias, respectively. Meanwhile, FAZ was negatively correlated with central retinal thickness in the ERM group. The vessel area (p = 0.0017) and vessel percentage area (p = 0.044) were significantly greater in the ERM group. Conclusions: Changes observed in the superficial plexus in OCTA are related to the severity of metamorphopsia and can be further evaluated to support decision making regarding the surgical management of idiopathic ERM.

Abstract Background: To evaluate the foveal avascular zone (FAZ) and retinal structure in familial exudative vitreoretinopathy (FEVR).Methods: Twenty FEVR eyes with stage 1 or 2 disease and 20 control eyes were evaluated. The central retinal thickness (CRT), inner retinal thickness (IRT), surface retinal vessel density (SRVD), and deep retinal vessel density (DRVD) were measured using optical coherence tomography. The FAZ area was calculated using ImageJ software. The equivalent spherical value (SE) and axial length (AL) were measured. Results: The CRT (232±26.75 vs. 213.15 ± 16.138 μm; p=0.0003) and IRT (17.44±13.28 vs. 1.85 ± 5.696 μm; p=0.0005) were thicker in the FEVR group than in the control group. The surface FAZ area (0.26±0.1 vs. 0.33 ± 0.1 mm2; p=0.006) and the deep FAZ area (0.36±0.1 vs. 0.43 ± 0.1 mm2; p=0.037) were smaller in the FEVR group than in the control group. The SRVD values did not differ among the sectors, but the DRVD was higher in the FEVR group except for the inferior sector (superior, p=0.02; inferior, p=0.4; temporal, p=0.001; nasal, p=0.02). The SE and AL did not differ between the two groups. There was no correlation between the surface and deep layer FAZ area and age, CRT, SE, and AL. The surface, deep FAZ area, and IRT were correlated negatively (surface, r = −0.57, p=0.008; deep layer FAZ area, r = −0.5, p=0.02).Conclusion: Eyes with FEVR has a smaller FAZ because the inner retina with the vascular structure remained in the fovea.

Background: Cytomegalovirus retinitis (CMVR) is a crucial blind-causing disease of AIDS-related ocular opportunistic infection. The CMVR lesions produced retinal necrosis. It is not entirely clear whether CMVR eyes without macular-involved necrotic lesions may have subtle macular damage. In this study, we conducted a cross-sectional study using optical coherence tomography angiography (OCTA) to evaluate macular microvasculature and structure in eyes with AIDS-related CMVR.Methods: Acquired immune deficiency syndrome (AIDS)-related CMVR patients (active and inactive CMVR) and healthy controls treated in the Department of Ophthalmology, Beijing Youan Hospital, Capital Medical University between August 25, 2019, and October 18, 2019, were recruited. All OCTA parameters, including the foveal avascular zone (FAZ), retinal vessel density (VD), choroidal vascularity index (CVI), retinal thickness, and choroidal thickness, were compared between groups after the signal strength was corrected.Results: Signal strength in the 3 × 3 and 6 × 6 mm scan patterns was significantly weaker in the inactive CMVR group than in the control group (both p < 0.001). After adjusting for signal strength, heterogeneity in the central fovea and parafoveal quadrants was present with a shift toward lower macular chorioretinal vasculature, decreased full choroidal thickness, and thicker retinal thickness in the active and inactive CMVR groups. The retinal nerve fiber layer (RNFL) and inner nuclear layer (INL) were significantly thicker in the active and inactive CMVR groups than in the control group (all p < 0.05). For photoreceptor-retinal pigment epithelium (PR-RPE) thickness, no significant differences were found in any quadrant between groups. Foveal avascular zone areas were not significantly different among the three groups (p = 0.053).Conclusions: Subtle macular structure and microvasculature damage still existed in CMVR eyes without macular-involved necrotic lesions. The results of our study are helpful for a deep understanding of the damage caused by CMVR.

Purpose: To investigate the correlation between outer and inner retina optical coherence tomography (OCT) biomarkers and visual acuity in patients with idiopathic epiretinal membrane (iERM) and identify which of them may be predictive of visual function. Methods: A retrospective cross-sectional single-center study was conducted that included patients diagnosed with iERM. Spectral domain OCT images were obtained and assessed qualitatively and quantitatively. The association of OCT parameters with best corrected visual acuity was analyzed. Results: Charts of 97 eyes of 97 patients were reviewed. Central foveal thickness, maximal retinal thickness (MRT), photoreceptor outer segment length, outer foveal thickness, ganglion cell-inner plexiform layer complex thickening, inner retinal thickness and inner retinal layer irregularity index were among the major outcome measures. OCT scans were also assessed for the presence of cotton ball sign, ellipsoid zone disruption, ectopic inner foveal layer, disorganization of retinal inner layers (DRIL), intraretinal fluid, subretinal fluid (SRF) and epimacular membrane rip. Univariate analysis showed statistically significant association between all the aforementioned parameters with worse vision, except for cotton ball sign and SRF. Multivariate analysis found that MRT and severe DRIL were strongly correlated with worse vision ( p < 0.001). Conclusion: MRT and severe DRIL should be considered as negative prognostic factors for visual acuity.

The size and shape of the foveal avascular zone (FAZ) have a strong positive correlation with several vision-threatening retinovascular diseases. The identification, segmentation and analysis of FAZ are of great significance to clinical diagnosis and treatment. We presented an adaptive watershed algorithm to automatically extract FAZ from retinal optical coherence tomography angiography (OCTA) images. For the traditional watershed algorithm, “over-segmentation” is the most common problem. FAZ is often incorrectly divided into multiple regions by redundant “dams”. This paper analyzed the relationship between the “dams” length and the maximum inscribed circle radius of FAZ, and proposed an adaptive watershed algorithm to solve the problem of “over-segmentation”. Here, 132 healthy retinal images and 50 diabetic retinopathy (DR) images were used to verify the accuracy and stability of the algorithm. Three ophthalmologists were invited to make quantitative and qualitative evaluations on the segmentation results of this algorithm. The quantitative evaluation results show that the correlation coefficients between the automatic and manual segmentation results are 0.945 (in healthy subjects) and 0.927 (in DR patients), respectively. For qualitative evaluation, the percentages of “perfect segmentation” (score of 3) and “good segmentation” (score of 2) are 99.4% (in healthy subjects) and 98.7% (in DR patients), respectively. This work promotes the application of watershed algorithm in FAZ segmentation, making it a useful tool for analyzing and diagnosing eye diseases.
