SODIUM PROFILING AND ULTRAFILTRATION IN PATIENTS UNDERGOING PROGRAMMED HEMODIALYSIS: THERAPY OPTIMIZATION

The objective of the study is to evaluate the effect of sodium profiling and ultrafiltration on the clinical and functional status of patients undergoing programmed hemodialysis (HD). Materials and Methods. The study consisted of two stages. During the first stage, a group of patients (n=25) undergoing HD was formed. The follow-up period lasted 20 months. Evaluation of blood pressure levels, interdialytic weight gain and laboratory parameters was carried out taking into consideration five variants of sodium profiling and ultrafiltration. During the second stage a comparative analysis of patients’ clinical and functional status according to the profiling option (3 and 5) was conducted in the group (n=20). Results. During the first stage, the authors found out that profiles 3 and 5 were desirable for blood pressure stabilization, interdialytic weight gain loss and improvement of laboratory parameters. The second stage showed that using profile 5 in case of adequate hemodialysis contributes to improving clinical and laboratory status and reducing concomitant drug therapy in 63.3% of patients. Conclusions. Under profiling patients undergoing hemodialysis demonstrate blood pressure stabilization, interdialytic weight gain loss and improvement in such blood values as total protein, albumin and hemoglobin. Pasien CKD (Chronic Kidney Diseases) yang menjalani hemodialisis sering mengalami komplikasi gangguan hemodinamik baik itu hipertensi maupun hipotensi intradialisis. Salah satu penyebab yang paling sering adalah peningkatan IDWG (Interdialytic Weight Gain). Tujuan penelitian ini adalah untuk menganalisis hubungan antara IDWG (Interdialytic Weight Gain)dengan perubahan tekanan darah intradialisis pada pasien CKD di Ruang Hemodialisis RSUP Sanglah Denpasar. Metode penelitian yang digunakan adalah analitik korelasi dengan pendekatan cross sectional. Teknik sampel yang digunakan adalah purposive sampling dengan jumlah sampel sebanyak 80 responden. Analisis statistik dengan menggunakan spearmans rank. Hasil penelitian didapatkan dari 80 responden sebagian besar mempunyai interdialytic weight gain dalam kategori ringan (73,8,0%) dan tekanan darah intradialisis dalam batas normal (65,0%). Hasil uji statistik menunjukkan p value 0,001 (<0,05). Kesimpulan dari penelitian ini bahwa ada hubungan yang signifikan antara interdialytic weight gain dengan perubahan tekanan darah intradialisis pada pasien chronic kidney diseases di Ruang Hemodialisis RSUP Sanglah Denpasar. Berdasarkan hasil penelitian ini bahwa perawat diharapkan dapat meningkatkan pemberian edukasi secara berkala pada pasien hemodialisis reguler untuk meningkatkan kepatuhan pasien pada diet dan pembatasan intake cairan.Chronic Kidney Diseases (CKD) patients who undergo hemodialysis often experience complications of haemodynamic disorders, both hypertension and intradialisis hypotension. One of the most frequent causes of increasing is IDGW (Interdialytic Weight Gain). The purpose of this study was to determine the correlation between interdialytic weight gain and changes in intradialysis blood pressure among patients with chronic kidney diseases in Haemodialysis Roomat Sanglah Hospital Denpasar. The research method used is analytic correlation with cross sectional approach. The sample technique used was purposive sampling with a total sample of 80 respondents. Statistical analysis using spearmans rank statistic test. The results obtained from 80 respondents most mostly interdialytic weight gain in the mild category (73.8.0%) and intradialisis blood pressure within normal limits (65.0%). The result of statistic test shows that p value 0,001 (<0,05). The conclusion of this study is that any significant correlation between interdialytic weight gain with changes in intradialysis blood pressure among patients with CKD at Haemodialysis Room of Sanglah Hospital Denpasar. Suggestions for nurses to increase the provision of regular education in regular haemodialysis patients to improve patient’s adherence to diet and limit fluid intake.

Interdialytic weight gain in patients with hemodialysis depends on fluid, salt intake in interdialytic period and compliance of dialysis patients. In end stage renal disease(ESRD) patients due to decreased and absent residual urine output there is salt and water retention in body which leads to interdialytic weight gain. Overhydration and IDWG is assumed to be the main cause of pre hemodialysis raised blood pressure(BP) in ESRD patients. Here the relationship between IDWG and pre HD blood pressure is studied in 40 hemodialysis patients. We recorded pre HD BP and IDWG in four subsequent hemodialysis sessions for each patient during the period of 2 weeks in patients who are on twice weekly hemodialysis. so, in total we studied 160 hemodialysis sessions. Mean of four pre HD BP is compared with mean of four IDWG for each patient. Then correlation between 40 mean IDWG and 40 mean pre HD BP is studied and also correlation between IDWG and pre HD blood pressure during 160 hemodidlysis sessions studied individually.

Aims: This study was aimed at comparing the incidence of arterial hypertension and blood pressure (BP) variance in hospital and out-of-hospital hemodialysis (HD) patients during HD sessions. Methods: A cross-sectional study was conducted for 1 week at all the HD centers in Dalmatia, Croatia. The pre-, intra-, and post-dialysis BP values were collected for 3 consecutive HD sessions per patient. Results: Of the 399 subjects, 73.9% were hypertensives, who showed higher interdialytic weight gain compared to the normotensives (2.58 vs. 2.40). Hospital and out-of-hospital HD patients received identical antihypertensive therapies, except that beta blockers were more frequently administered to out-of-hospital HD patients. Higher pre-, intra-, and post-dialysis BP values were recorded in patients at out-of-hospital HD centers. Conclusion: The differences in BP variability and antihypertensive therapies administered to hospital HD patients as compared to out-of-hospital HD patients may reflect differing approaches by the nephrologists at these centers.
