In the last two decades, 308-nm excimer laser has been increasingly recognized as a therapeutic alternative for several dermatological conditions, being currently FDA approved for the treatment of localized vitiligo and moderately severe localized psoriasis unresponsive to topical treatments. We describe the case of a 17-year-old with a recalcitrant form of dermatitis occupying the entire perioral region, previously unresponsive to several treatments, who was treated with 308-nm excimer laser with an excellent result. 308-nm monochromatic excimer laser has several advantages over other types of phototherapy, including lower UV dose exposure, shorter courses of therapy and a better sparing of adjacent tissue. Although infrequently used, 308-nm excimer laser certainly has a vast potential in Dermatology, particularly regarding recalcitrant and localized inflammatory conditions, such as the one we present.

Currently, dental implants are a very frequent therapeutic alternative for replacing missing teeth. However, they are not exempt from developing complications of infectious origin. Therefore, this case report describes a 67-year-old female patient presenting infectious complications caused by suture rupture. Surgery combined with therapeutic management with antibiotics was performed, allowing preservation of the osseointegration of the implants in the lower arch. Within the limitations of this study, it was shown that more research is needed to determine the success and survival of implants presenting complications due to infections during the osseointegration process.

Oral Lichen Planus (OLP) is considered a potentially malignant lesion (PML), although its rate of transformation is controversial. New treatments have been introduced recently, for example calcipotriene (D3 vitamin analog). Its incremental action with glucocorticoids is observed and isolated lip lesions may respond positively to combined topical therapy. Thus, objective of this case report is to show a therapeutic alternative for isolated and persistent OLP lip lesions. An 18-year-old teenager was referred to evaluate an erythematous lower lip lesion with approximately 10 years of evolution. The inspection revealed multiple erythematous areas surrounded by thin whitish streaks located in the vermilion of the lower lip. No abnormalities in the skin, nails, scalp or other areas of oral mucosa were observed. The incisional biopsy was performed and the microscopic exam showed areas of basal layer degeneration with intense mononuclear inflammatory infiltrate banded and predominantly subepithelial. The therapeutic proposal was topical application of Daivobet®, a combination of topical corticosteroids and vitamin D derivative. The lesion had remission after the fifteen days, medication was suspended and indicated when there was a relapse. During follow-up, no recurrences or complications were observed. That combination therapy may be a new approach in treating OLP.

Heterotopic pregnancy is defined as the coexistence of an intrauterine pregnancy (UGI) and an ectopic pregnancy (EUS), regardless of its location. It is a rare pathology. Here, we report the case of a 29-year-old patient who had a heterotopic in spontaneous pregnancy complicated by hemoperitoneum at 7 weeks of amenorrhea. A laparotomy with salpingectomy was performed. The result was favourable with a live birth with 37 SA of the GIU. A heterotopic pregnancy must always be eliminated in the presence of acute pelvic pain in the pregnant woman. Laparotomy is a therapeutic alternative if laparoscopy is not available.

Purpose Misophonia is a sound tolerance disorder condition in certain sounds that trigger intense emotional or physiological responses. While some persons may experience misophonia, a few patients suffer from misophonia. However, there is a dearth of literature on audiological assessment and management of persons with misophonia. The purpose of this report is to discuss the assessment of misophonia and highlight the management option that helped a patient with misophonia. Method A case study of a 26-year-old woman with the complaint of decreased tolerance to specific sounds affecting quality of life is reported. Audiological assessment differentiated misophonia from hyperacusis. Management included retraining counseling as well as desensitization and habituation therapy based on the principles described by P. J. Jastreboff and Jastreboff (2014). A misophonia questionnaire was administered at regular intervals to monitor the effectiveness of therapy. Results A detailed case history and audiological evaluations including pure-tone audiogram and Johnson Hyperacusis Index revealed the presence of misophonia. The patient benefitted from intervention, and the scores of the misophonia questionnaire indicated a decrease in the severity of the problem. Conclusions It is important to differentially diagnose misophonia and hyperacusis in persons with sound tolerance disorders. Retraining counseling as well as desensitization and habituation therapy can help patients who suffer from misophonia.
