Gambaran Parameter Optical Coherence Tomoghraphy Papil dan Hubungannya dengan Fungsi Penglihatan pada Pasien Non-Arthreritic Anterior Ischemic Optic Neuropathy di FKUI-RSCM Kirana

Tujuan : Untuk menggambarkan parameter diskus optik dan retinal nerve fiber layer (RNFL) pada pasien non arteritic anterior ischemic optic neuropathy (NAION) dan untuk menggambarkan korelasinya dengan defek lapang pandang dan tajam penglihatan. Metode: 27 pasien NAION disertakan pada penelitian ini. Pemeriksaan berikut dilakukan secara potong lintang: Humphrey HFA II-i 750 24-2 threshold , tajam penglihatan dengan koreksi (TPDK) dengan Snellen chart dan optical coherence tomography (OCT) StratusTM fast optic disc dan fast RNFL 3.4mm. Hasil : Kami menemukan 14 mata (25,9%) dengan diskus optik edema, 21 mata (38,9%) dengan diskus optik atrofi dan 19 mata dengan diskus optik normal. Disk Area dan Rim cross sectional area dapat membedakan kondisi diskus edema, atrofi, dan normal (DA: 3,52 ± 1,16; 2,73 ± 0,55; 2,59 ± 0,44; p = 0,01) (RCSA : 2,44 ± 1,34; 1,52 (0,94-3,02); 1,83 ± 0,34; p = 0,014) masing-masing. Ketebalan RNFL di semua kuadran juga dapat membedakan morfologi diskus. Tidak ada korelasi yang ditemukan antara parameter diskus optik-RNFL dengan defek lapang pandang (LP). Kami menemukan korelasi sedang antara ketebalan RNFL kuadran temporal dengan TPDK (r -0,433; p = 0,05). Kesimpulan : Parameter diskus optik dan RNFL dengan OCT dapat digunakan untuk membedakan morfologi diskus optik. Ketebalan kuadran temporal RNFL adalah prediktor baik tajam penglihatan sentral pada pasien dengan NAION karena fungsi makula.

Aims. To compare optical coherence tomography angiography (OCT-A) retinal vasculature measurements between normal and optic atrophy after nonarteritic anterior ischemic optic neuropathy (NAION) subjects. Design. This prospective observational study was conducted between July 2015 and August 2016 at the ophthalmology outpatient department of a referral center in Taiwan. Peripapillary (4.5 × 4.5 mm) and parafoveal (6 × 6 mm) OCT-A scans were acquired. Measurements of the peripapillary region were obtained in two areas: (1) circumpapillary vessel density (cpVD) and (2) whole enface image vessel density (wiVD). Results. 13 participants with optic atrophy after NAION had lower peripapillary vessel density than the 18 age-matched participants in the healthy control (HC) group (p<0.001 for both cpVD and wiVD). However, the parafoveal vessel density was not significantly different between the two groups (p=0.49). The areas under the receiver operating characteristic curve for the HC and NAION eyes were 0.992 for cpVD and 0.970 for wiVD. cpVD and wiVD were significantly correlated with the average retinal nerve fiber layer thickness (p<0.001 for both). Conclusion. Peripapillary retinal perfusion is significantly decreased in optic atrophy after NAION. OCT-A may aid in the understanding of structure-function-perfusion relationships in NAION.

Background. To analyze the blood flow changes of radial peripapillary capillaries (RPCs) and macula with time procession in patients with nonarteritic anterior ischemic optic neuropathy (NAION) by optical coherence tomography angiography (OCT-A). Methods. A total of 21 affected eyes and 19 unaffected eyes from 21 NAION patients were included. Assessments of BCVA, CFP, SD-OCT, and OCT-A were performed on NAION patients at enrollment and at 1-2 weeks, 1-2 months, and 3–6 months after enrollment. Measures of the thickness of the peripapillary retinal nerve fiber layer (wRNFL) and macular ganglion cell complex (wGCC) of the whole image in SD-OCT, vessel density of the RPC (wRPC) and superficial and deep vascular complexes (wSVD, wDVD) in the whole image of OCT-A, and their superior- and inferior-hemi values (s/iRNFL, s/iGCC, s/iRPC, and s/iSVD) were assessed. Results. Compared to unaffected control eyes, wRPC ( p ≤ 0.001 ) was significantly lower in affected eyes at baseline, and there was no significant difference in wSVD ( p > 0.05 ). The wRPC and wSVD values of affected eyes were significantly decreased at follow-up time points of 1–2 and 3–6 months compared to baseline ( p = 0.001 , p ≤ 0.001 ; p ≤ 0.001 , p ≤ 0.001 ). The sRPC values were significantly lower than iRPC at 1-2/3–6 months ( p = 0.016 , p = 0.013 ), and sSVD values were lower than iSVD at 1-2 months ( p = 0.010 ). Statistically significant correlations were found between wRPC and wRNFL values at 3–6 months (r = 0.626, p = 0.022 ), between wSVD and wGCC at 1-2 weeks and 1-2 months (r = 0.570, r = 0.436; p = 0.007 , p = 0.048 ). Conclusion. OCT-A revealed a sectorial reduction in vessel density in the RPC and macula with the disease progression of NAION from acute to atrophic stages, a classification associated with structural deficits.
