scholarly journals Neurointerventional and Surgical Management of a Carotid Body Tumor: Case Report

Carotid body tumors are a rare type of head and neck tumor that can be safely resected with the help of pre-operative neuro-interventional embolization. This adjunct intervention helps reduce blood loss and to maintain a near bloodless surgical field that allows for good visualization of surrounding neurovascular elements. With the improvements of neuroimaging in the angiography suite and technologies surrounding neuro-interventional procedures, the complications of this procedure have decreased dramatically compared to what was reported in the 1980s-2000s.

Background: Carotid body paragangliomas are rare neuroendocrine neoplasms of chromaffin negative glomus cells. This case report explains an atypical case with unusual presentation and treatment. Case Report: A healthy smoker technician by profession was brought to the emergency room (ER) with coprolalia. The general physical exam did not reveal any information. His history revealed unilateral tinnitus and odynophagia leading to a consultation by the neurologist with head imaging. Acoustic neuroma was ruled out and the caregiver was asked to elaborate the events mentioned in the history and a psychiatric examination was done. The personality changes were evaluated by the psychiatrist that showed overlapping of delirium and depression. The patient was further examined by a vascular surgeon. After careful revisiting of the history, examination and indication of tender mass in the neck by the patient's vascular surgeon, the diagnosis of carotid body paraganglioma was made which was followed by surgical resection for treatment. Conclusion: Carotid Body Paragangliomas are very vascular structures and their manipulation in a surgery setting requires expertise. This case presented with personality changes and tinnitus, a very less likely event to occur in a carotid body tumor. The present care report, thus adds on to the literature of carotid body tumors and its presenting symptoms.

A one-year-old entire male Caucasian Mountain Dog underwent tibial plateau levelling osteotomy. During surgery the tibialis cranialis artery and vein were inadvertently severed and blood loss reached 20 per cent in 30 minutes. Blood from the surgical field was collected and an autotransfusion was performed using a novel ultrafiltration device (Hemosep, Advancis Surgical) without major complications. This case report describes the use the Hemosepand discusses the different techniques available for autotransfusion. To the authors’ knowledge, this is the first case report of autotransfusion using an ultrafiltration device to control acute haemorrhage in veterinary medicine.

A case of gangliocytic paraganglioma of the duodenum, the 25th in world medical literature, is reported. The histologic appearance consists of epithelioid cell nests resembling « Zellballen » of carotid body tumors, together with gangliocyte-like elements. Ultramicroscopically, the cytoplasm of the cells contains round electron-dense granules or a large number of filaments. Occasional cells contain both granules and filaments.
