We report a rare case of radial dislocation of the extensor tendon over the metacarpophalangeal (MCP) joint of the thumb. Ulnar dislocation has been reported, but previous reports have not mentioned radial dislocation of the extensor tendon in the thumb. Radial dislocation of the extensor tendon of the thumb gradually progressed after cerebral hemorrhage and the patient could not extend MCP joint of the thumb. To resolve difficulty in extending the MCP joint, surgical centralization of the extensor tendon and partial resection of the flexor pollicis brevis (FPB) and abductor pollicis brevis (APB) insertions was performed. The patient recovered function of thumb extensors.

Abstract Syringomyelia has been described frequently in association with various abnormalities of the skull base but rarely with tumors of the skull base. Syringomyelia as a postoperative sequela of surgery of the skull base has not been reported. The authors describe a case of cervical syringomyelia developing after the partial resection of a chordoma and combined conventional and proton beam irradiation. The possible mechanisms causing this abnormality are discussed.

The authors of this article present a case of a woman diagnosed with a vaginal melanoma who developed paraneoplastic syndrome (PNS) soon after diagnosis. A review of the literature regarding PNSs in gynecological malignancies is also described in this article. To our knowledge, this is the first reported case of paraneoplastic cerebellar degeneration with opsoclonus myoclonus secondary to a vaginal melanoma. In addition, our patient had an unusually acute progression to pancerebellar symptoms over the course of 3 weeks. Her paraneoplastic symptoms improved significantly after partial resection of the melanoma.
