Correction of a Class III Malocclusion with All-on-4 Concept Implant Supported Full Mouth Rehabilitation: Clinical Case Report

This case report describes the correction of a skeletal Class III malocclusion by performing alveoloplasty and All-on-four styletitanium/acrylic (hybrid) fixed/detachable implant supported prosthesis.  A 76-year-old Hispanic,female patient with a noncontributory medical history presented to the clinic with a chief complaint of “I cannot wear my upper partial denture, it keeps on breaking”. Intraoral examination revealed significant inter-arch Class III relation, supra-erupted mandibular dentition, and significant loss of the vertical dimension of occlusion. Surgical treatment included edentulation of the patient in addition to a significant mandibular alveoloplasty in order to correct the patient’s malocclusion prosthetically. Through proper planning and execution, a skeletal Class III relationship was corrected with an implant supported All-on-four maxillary/mandibular prostheses. An edge-to-edge incisal relationship was achieved along with an appropriate vertical dimension of occlusion. The preplanning protocol allowed determination of the required alveoloplasty in order to obtain sufficient restorative space to allow for the correction of the skeletal malocclusion prosthetically.

Loss of occlusal vertical dimension is a frequently observed problem in the middle aged and aged population who intends to retain their natural dentition. This type of patient has very few treatment options one of which is overdenture. In this case report, a patient was treated with overlay removable partial denture for restoring the lost vertical dimension. This was provided as interim prosthesis. The follow up was done to observe the fulfillment of desired results. The clinical and laboratory procedure is described briefly. The treatment used for this case is cost effective and conservative solution for esthetic and functional rehabilitation. Since the material is acrylic resin, long term wear resistance is crucial for success of treatment. ABSTRACT A 66-year-old man visited our department with the chief complaint of poor esthetics due to loss of a fixed partial denture in a maxillary incisor tooth and difficulty in mastication due to an ill-fitting, mandibular removable partial denture. An analysis of the occlusal vertical dimension resulted in a diagnosis of infraocclusion. After increasing vertical dimension with a provisional restoration and treatment denture which covered the attritioned surface, a definitive prosthesis was applied. Untreated loss of molars and severe attrition of the remaining teeth caused infraocclusion in this patient, which increased occlusal loading on the incisor teeth and induced detachment of the prosthesis in the maxillary incisors. Esthetic appearance was improved and masticatory dysfunction restored by increasing vertical dimension and oral rehabilitation. Mildly invasive prosthetic treatment involved application of a removable partial denture which covered the attritioned surface, not only infraocclusion but masticatory dysfunction and poor esthetics were also treated. The management of tooth wear has been a subject of increasing interest from both preventive and restorative points of view. This paper describes the full mouth rehabilitation of a 63-year-old bruxer man with a severely worn dentition and other dental problems including unsuitable restorations and several missing teeth. The treatment entailed using cast posts and cores, metal-ceramic restorations, and a removable partial denture. As with the treatment procedure of such cases, equal-intensity centric occlusal contacts on all teeth and an anterior guidance in harmony with functional jaw movements were especially taken into account.

Abstract Aim This clinical report describes the occlusal rehabilitation of a partially edentulous patient with lost vertical dimension. Background The patient did not want a removable partial denture. Implants, fixed restorations with occlusal plane correction, were used to restore the anterior and posterior support and treat the severely worn dentition. However, a partially edentulous patient may be unable to recover normal function, esthetics, comfort, or phonetics with a conventional removable dental prosthesis. The use of dental implants–supported fixed prosthesis offers a multitude of benefits over a tooth–soft tissue supported removable partial denture prosthesis. Summary This clinical report describes an organized approach to an occlusal reconstruction of a severely worn dentition, partially edentulous patient with fixed restorations, implants, occlusal plane correction, and recovery of the vertical dimension. The treatment offered the patient improved esthetics and the restoration of oral function. Clinical Significance The main indications for implant-supported restorations in a partially edentulous patient are the elimination of the freeend distal extension and the benefit from fixed restorations. Success with implant-supported prosthodontics needs the same or more attention to detail and careful treatment planning for conventional fixed prosthodontics; a team approach is recommended.
