Loss of occlusal vertical dimension is a frequently observed problem in the middle aged and aged population who intends to retain their natural dentition. This type of patient has very few treatment options one of which is overdenture. In this case report, a patient was treated with overlay removable partial denture for restoring the lost vertical dimension. This was provided as interim prosthesis. The follow up was done to observe the fulfillment of desired results. The clinical and laboratory procedure is described briefly. The treatment used for this case is cost effective and conservative solution for esthetic and functional rehabilitation. Since the material is acrylic resin, long term wear resistance is crucial for success of treatment. The Aesthetic

The various treatment options for the aesthetic and functional rehabilitation of partially edentulous patients are conventional fixed partial dentures, implant supported fixed partial dentures and removable partial dentures. In some cases, removable partial dentures may be the only choice which is available other than implants and fixed partial denture. Removable cast partial dentures are used as definitive removable prostheses when indicated, but location of clasps is not aesthetically pleasant. So, when patient is concerned about aesthetics, flexible partial dentures which is aesthetically superior may be considered. But for the success of flexible removable partial denture, proper diagnosis, treatment planning, insertion, wearing and maintenance of this prosthesis is very important.

Treatment options for patients with severe attrition resulting in reduced occlusal vertical dimension are often limited to fixed prosthesis to reestablish proper occlusal vertical dimension and functional occlusion. In some cases such as when there are limited finances, minimal esthetic concerns, and medical considerations fixed prosthesis may not be the ideal treatment option. Overlay removable partial dentures (ORPDs) can be used as a provisional or interim prosthesis as well as permanent prosthesis in these cases. While ORPDs can provide a reversible and relatively inexpensive treatment for patients with a significantly compromised dental status, there is not much scientific evidence in the literature on ORPDs. Most studies published on ORPDs to date are primarily reviews and clinical reports. In this article, literatures on ORPDs are summarized and a patient treated with interim and permanent ORPDs is presented. This article reviews previously published literatures on the use of ORPDs. Indications, advantages and disadvantages are discussed. Treatment protocol with an example of the prosthodontic treatment of a patient with severely worn dentition with an interim ORPD and later a permanent ORPD are discussed in details.

Tooth wear is a common problem seen in population and poses restorative challenge to a dentist as well. Extensive dental procedures are usually required for the treatment of tooth wear associated with loss of occlusal vertical dimension. An overlay removable partial denture provides conservative treatment option for patients with moderate to severe worn down dentition and partial edentulism. It is designed in such a way that some part of denture component also covers the occlusal surface of remaining teeth and replace the remaining teeth to achieve a functionally stable occlusion. This case report is about a patient who was given cast metal overlay upper and lower denture for a correction of lost vertical dimension of occlusion, replacement of few missing teeth and for the prevention from further tooth wear. Introduction: Rehabilitation with the use of overlay-type removable prostheses is indicated in cases of more severe occlusal dysfunction. This type of treatment meets the need for more immediate results, providing aesthetic improvement while the rehabilitation work is being done, while promoting the gradual rehabilitation of occlusal function, re-adjusting the intermaxillary relationship. In addition, the patient can anticipate the final result of the treatment beforethe definitive prosthesis is made. Objective: To show a clinical case of planning and execution of removable overlays. Case report: JCF patient had occlusal dysfunction and presented as main complaint the aesthetic and functional deficiency. He used a removable partial denture (PPR) only on the upper arch. The existing prosthesis did not present stability due to the loss of some dental elements, pillars of the PPR. Two overlays (upper and lower) were indicated because the patient had good periodontal conditions in the remaining dentalelements that were in a favorable position. Necessary exodontia, adequacy of the buccal environment with sub and supragingival scaling and the fabrication of overlays were performed. Conclusion: The use of temporary overlays can provide the necessary time for the patient to adapt to the new occlusal conformation in the period in which the restorations and definitive prostheses are expected to be performed. The use of overlays presented a satisfactoryresult for the proposed treatment, since it promoted the functional rehabilitation, restored aesthetic, even temporarily, and motivated the patient to continue the treatment. According to the patient, there were significant improvements in chewing quality and self-esteem.

<p class="AbstractContent">Restoring occlusal plane and occlusal vertical dimension (OVD) in patient with existing indirect restorations who must wear removable prostheses may be a challenge to the dental operator. Onlay removable partial dentures (RPD) are used to re-establish the occlusion in conjunction to replace missing teeth without having to remove the existing indirect restoration. This case report described prosthetic rehabilitation with onlay RPD in patients who were partially edentulous and has reduced OVD. The treatment involved a set of provisional RPD to re-establish the OVD, as well as to evaluate the function, esthetic and speech of the patient, followed by definitive onlay RPD. The mandibular onlay RPD was used to provide stable occlusion and to correct the uneven occlusal surface on the abutment teeth. This treatment able to improve patient’s chewing function through a simple removable prosthesis and conservative on the existing restorations.</p>

ABSTRACT A 66-year-old man visited our department with the chief complaint of poor esthetics due to loss of a fixed partial denture in a maxillary incisor tooth and difficulty in mastication due to an ill-fitting, mandibular removable partial denture. An analysis of the occlusal vertical dimension resulted in a diagnosis of infraocclusion. After increasing vertical dimension with a provisional restoration and treatment denture which covered the attritioned surface, a definitive prosthesis was applied. Untreated loss of molars and severe attrition of the remaining teeth caused infraocclusion in this patient, which increased occlusal loading on the incisor teeth and induced detachment of the prosthesis in the maxillary incisors. Esthetic appearance was improved and masticatory dysfunction restored by increasing vertical dimension and oral rehabilitation. Mildly invasive prosthetic treatment involved application of a removable partial denture which covered the attritioned surface, not only infraocclusion but masticatory dysfunction and poor esthetics were also treated.
