ABSTRACT A 66-year-old man visited our department with the chief complaint of poor esthetics due to loss of a fixed partial denture in a maxillary incisor tooth and difficulty in mastication due to an ill-fitting, mandibular removable partial denture. An analysis of the occlusal vertical dimension resulted in a diagnosis of infraocclusion. After increasing vertical dimension with a provisional restoration and treatment denture which covered the attritioned surface, a definitive prosthesis was applied. Untreated loss of molars and severe attrition of the remaining teeth caused infraocclusion in this patient, which increased occlusal loading on the incisor teeth and induced detachment of the prosthesis in the maxillary incisors. Esthetic appearance was improved and masticatory dysfunction restored by increasing vertical dimension and oral rehabilitation. Mildly invasive prosthetic treatment involved application of a removable partial denture which covered the attritioned surface, not only infraocclusion but masticatory dysfunction and poor esthetics were also treated. <p class="AbstractContent">Restoring occlusal plane and occlusal vertical dimension (OVD) in patient with existing indirect restorations who must wear removable prostheses may be a challenge to the dental operator. Onlay removable partial dentures (RPD) are used to re-establish the occlusion in conjunction to replace missing teeth without having to remove the existing indirect restoration. This case report described prosthetic rehabilitation with onlay RPD in patients who were partially edentulous and has reduced OVD. The treatment involved a set of provisional RPD to re-establish the OVD, as well as to evaluate the function, esthetic and speech of the patient, followed by definitive onlay RPD. The mandibular onlay RPD was used to provide stable occlusion and to correct the uneven occlusal surface on the abutment teeth. This treatment able to improve patient’s chewing function through a simple removable prosthesis and conservative on the existing restorations.</p>

Loss of occlusal vertical dimension is a frequently observed problem in the middle aged and aged population who intends to retain their natural dentition. This type of patient has very few treatment options one of which is overdenture. In this case report, a patient was treated with overlay removable partial denture for restoring the lost vertical dimension. This was provided as interim prosthesis. The follow up was done to observe the fulfillment of desired results. The clinical and laboratory procedure is described briefly. The treatment used for this case is cost effective and conservative solution for esthetic and functional rehabilitation. Since the material is acrylic resin, long term wear resistance is crucial for success of treatment. Oral rehabilitation of partially edentulous arches requires careful treatment planning before any prosthodontic intervention. The connection of the metal framework of fixed (fixed dental prosthesis (FPD)) and removable partial denture using adhesive attachments is a good alternative prosthetic option when solely fixed prosthesis (FPD or implant) cannot be used due to anatomical limitation. Attachments are the tiny interlocking devices that act as a hybrid link to join removable prosthesis to the abutment and direct the masticatory forces along the long axis of the abutment. This joint acts as a non-rigid stress breaker, which helps in distributing the occlusal load. Precision and semiprecision attachment have always been bordered by an aura of mystery due to technique sensitive procedure and lack of knowledge. The following case describes a combined contemporary and conventional approach and treatment sequence with the use of attachments for the rehabilitation of partially edentulous arches.

A dimensão vertical de oclusão é descrita como a altura do terço inferior da face enquanto existe o máximo de contatos posteriores entre os dentes, e, sua diminuição é ocasionada pela perda total ou parcial de dentes posteriores. O objetivo do presente trabalho é relatar um caso clínico de restabelecimento da dimensão vertical de oclusão através da confecção de prótese parcial removível overlay. Paciente compareceu à clínica odontológica da Universidade Nilton Lins queixando-se da aparência do seu rosto, aspecto do sorriso, dentes desgastados e dificuldade de alimentar-se. Ao exame extraoral, observou-se aprofundamento do sulco-nasogeniano, diminuição de altura do terço inferior da face e ausência de pontos de gatilho. Ao exame intraoral constatou-se extensa reabsorção óssea no arco inferior, ausência de todos os dentes posteriores do arco inferior, e, a presença de desgastes e fraturas dentárias em ambos os arcos. Após os exames, constatou-se a presença de desarmonia oclusal e diminuição da dimensão vertical de oclusão. Foram realizados planejamento e enceramento de diagnóstico com o objetivo de restabelecer a oclusão ideal perdida. Optou-se então pela confecção de próteses temporárias removíveis overlay com encaixe sobre a estrutura dentária remanescente. Overlay removable partial denture can be used as an interim prosthesis for the patients who have lost their occlusal vertical dimension due to moderate to severe attrition resulting in functional and esthetic problems. This prosthesis is believed to allow neuromuscular adjustments to a newly adjusted VDO and also to observe the function and esthetics. ORPDs provide reversible and relatively inexpensive treatment for the patients who have limitations. In this case report, a patient was treated with overlay removable partial denture for restoring the lost vertical dimension. This article reviews indication, advantage, disadvantages of ORPDs. Though there are esthetic and functional limitations with potential risk of material fracture, all patients with severely worn dentition with decreased VDO and missing teeth should be offered this treatment option. This case report presents a 19-year-old male patient with hypohidrotic ectodermal dysplasia, having a chief complaint of multiple missing teeth. Atraumatic extraction of the teeth with hopeless prognosis was done, and teeth with grade 2 mobility were submerged using cast dowel and coping. Following this, incremental increase in the vertical dimension was made using removable flexible splint of two-millimeter thickness. After facebow transfer and making appropriate eccentric bite records to program the semiadjustable articulator, wax-up was done at the desired vertical dimension (VD). The upper arch was finally restored using a long-span fixed partial denture and lower arch using bilateral attachment (Rhein 83) retained cast removable partial denture as a definitive prosthesis. Therefore, in conditions like hypodontia or oligodontia caused due to ectodermal dysplasia, attachment retained removable partial denture may prove beneficial by effectively distributing the occlusal forces. In clinical scenarios where implant is not feasible or not opted by the patient, this combination treatment may be a viable option.
