Maxillary central incisor impactions occur infrequently.Their origins include various local causes, such as odontoma, supernumerary teeth, and space loss. Supernumerary and ectopically impacted teeth are asymptomatic and found during routine clinical or radiological examinations. The surgical exposure and orthodontic traction of impacted right central incisor after removal of odontomas is presented in this report. Objective: One of the most common types of developmental odontogenic cyst is the dentigerous cyst. It encloses the crown of the tooth and is attached at the cementoenamel junction. Although its association with mandibular molars is common, it is rarely associated with the maxillary central incisors. Discussion: The present case report describes an unusual occurrence of dentigerous cyst associated with the impacted permanent maxillary central incisor in an inverted position and showing dilaceration of the root. The cyst was enucleated along with the extraction of the impacted tooth. We have discussed clinical presentation, radiographic features and treatment modalities of this uncommon and rare presentation of this lesion. Conclusion: Trauma to the deciduous teeth should not be overlooked, since it can result in the development of a pathology which could indirectly affect the permanent successors.

This report demonstrates a simple endodontic solution to an interdisciplinary case of a patient with multiple gene deficiencies. An adolescent patient presented with an impacted premolar that could not be extracted due to high-risk conditions. A suspicious degree of radiolucency around the crown was clinically diagnosed as dentigerous cyst. The treatment dilemma and implementation are discussed. This case report offers a different treatment option for impacted teeth when significant differential diagnosis of a pericoronal lesion dictates treatment.
