Objective: to build a descriptive classification of premolar and molar supernumerary teeth (ST) when preparing the cone beam computed tomography (CBCT) report. The aim is also to share wide range of CBCT images in the open access publishing model. Material and methods: For our review we systematically searched for articles from PubMed with 1) free full texts on ST in molar and premolar area and using CBCT, and 2) articles providing with information on complications related with the presence of ST in molar and premolar area. We also added to our review studies providing with classic ST classifications in premolar and molar area. Results: We found 29 cases of ST, and we freely illustrated them with 84 figures. We separated our pictorial review in: 1) unilateral ST in the mandible, 2) unilateral ST in the maxilla, 3) unilateral undersized ST, 4) bilateral ST, 5) ST with additional features, and 6) cases with major hyperdontia. Conclusions: we build up the classification matrix for premolar and molar ST with 11 descriptors and 50 boxes. The descriptors were: 1) location if the ST crown in axial view, 2) vertical location of the cusp tip in relation with closest erupted tooth in coronal view, 3) shape, 4) distribution, 5) Position (in relation to normal tooth eruption) in sagittal view, 6) State of eruption of the ST in the sagittal view, 7) Follicle size measurement in sagittal view, 8) External root resorption of adjacent teeth by ST and its location in relation to the long axis of the involved tooth, 9) Internal resorption of ST, 10) Adjacent tooth complication, and 11) Damage to surrounding structures if ST removal. The open access figures from the literature illustrated 11 boxes. With our pictorial review we were able to illustrate 45 out of 50 boxes, and freely provide the readership with the most complete description of ST in premolar and molar area on CBCT than in previously published studies.

Supernumerary teeth may be encountered as an incidental finding on a radiograph. When impacted, they may be associated with clinical signs related to different problems such as failure of eruption, teeth displacement, root resorption or cystic lesions. They may occur in primary and permanent dentition, in both the maxilla and mandible and can be single or multiple in patients with syndromes. Mesiodens is the most commonly impacted tooth and appears between the central maxillary incisors in pediatric ages. Supernumerary teeth distal to the third molar are rare, usually impacted and referred to as a distomolar. A 46-year-old male consulted with the main complaint of pain on the left side of the maxilla. A panoramic radiograph revealed a right impacted maxillary fourth molar located posterior to the third molar associated with a pericoronal radiolucency. The supernumerary tooth was removed surgically under local anesthesia and the pericoronal lesion enucleated. Histopathological examination was consistent with the diagnosis of a dentigerous cyst associated with an impacted distomolar. Healing was uneventful, and the patients remained asymptomatic. The occurrence of a maxillary distomolar is rare and even rarer the association with a dentigerous cyst.

Supernumerary teeth and odontomas are obstacles for spontaneous tooth eruption and may result in impaction. The aim of the study is to present a conservative treatment approach for impacted teeth following surgical obstruction removal by reviewing three treatment modalities: surgery only, which involves the surgical removal of the obstruction and the spontaneous eruption; surgery with immediate traction, which includes surgery combined with immediate active orthodontic brace cementation and traction; and surgery with delayed traction, which combines a surgical procedure of obstacle removal and orthodontic brace cementation with follow-up for the spontaneous eruption. The first two modalities require orthodontic traction either by an additional surgical procedure for orthodontic brace cementation, or combined with the surgical obstacle removal. With the third approach, clinical follow-up is performed via connected ligature wire elongation applied during the surgical procedure for the spontaneous emergence of the impacted tooth. Active orthodontic traction is only employed if the tooth fails to erupt. The visual follow-up via wire elongation serves as a reference during the emergence of the impacted teeth and reduces the need for radiographic examination. The surgical-orthodontic approach saves both further surgery and orthodontics (spontaneous eruption) or further surgery (in failure to erupt).

Supernumerary teeth are defined as those in addition to the normal series of deciduous or permanent dentition. Supernumerary teeth are more prevalent among males and in the permanent dentition. The exact etiology of supernumerary teeth is unknown, they may occur anywhere in the oral cavity. They may appear as a single tooth or multiple teeth, unilaterally or bilaterally, erupted or impacted in maxilla or both jaws. Fourth molars, also called distomolars, are a variant in shape and number appearing as alterations during odontogenesis. They can be eumorphic or dismorphic, single or multiple, erupted or impacted, unilateral or bilateral, and can appear in both jaws. They are of unknown etiology, but there are several theories to justify these tooth alterations such as dental lamina duplication, its horizontal proliferation or its hyperactivity. Hereby, we are presenting a unique case report of presence of three distomolars in a single patient at maxillary left side and mandibular right and left side.

Background: Cleidocranial dysplasia (CCD) is a rare, autosomal dominant skeletal dysplasia with a prevalence of one per million births. The main causes of CCD are mutations in the core-binding factor alpha-1 (CBFA1) or runt-related transcription factor-2 (RUNX2), located at the 6p21 chromosomal region. RUNX2 plays important roles in osteoblast differentiation, chondrocyte proliferation and differentiation, and tooth formation. The disease is characterized by clavicular aplasia or hypoplasia, Wormian bones, delayed closure of cranial suture, brachycephalic head, maxillary deficiency, retention of primary teeth, inclusion of permanent teeth, and multiple supernumerary teeth. Materials and Methods: A 22-year-old girl suffering from cleidocranial dysplasia with short stature, narrow shoulders, craniofacial manifestations (short face, broad forehead, etc.) and dental anomalies (different lower dental elements under eruption, supernumerary and impacted multiple teeth, etc.) was examined at our service (Complex Operative Unit of Odontostomatology of Policlinico of Bari). RX Orthopantomography (OPG) and cone beam computed tomography (CBCT) were requested to better assess the position of the supernumerary teeth and their relationships with others and to evaluate the bone tissue. Results: Under eruption was probably caused by dental interferences with supernumerary teeth; hence, extractions of supernumerary upper canines and lower premolars were performed under general anaesthesia. Surgery outcome was excellent with good tissue healing and improvements in the therapeutic possibilities with future orthodontics. Conclusions: The objective of this article is to give an update about radiological, clinical, and molecular features of CCD and to alert the health team about the importance of establishing an early diagnosis and an appropriate treatment in these patients to prevent impacted teeth complications and to offer them a better quality of life.

Introduction: The presence of a developmental dental anomaly in maxillary incisors usually affects esthetic appearance. Therefore, gemination and supernumerary teeth are considered anomalies and need a careful and comprehensive diagnosis and treatment. Case Description: This clinical report presents a 10-year-old girl complaining of the esthetic appearance of her anterior maxillary teeth. Clinical and radiographical examinations revealed a maxillary geminated permanent left central incisor and a supplemental supernumerary permanent right lateral incisor. The treatment has been performed in different stages, including preventive, conservative, surgical, and orthodontic treatments. Thus, the large geminated incisor has been restored to its normal shape, and the problem of anterior incisor crowding resolved through alignment. Conclusion: Dental anomalies in the anterior segment are common and need time during treatment. Selective grinding for the large geminated incisor and the extraction of the supplemental tooth followed by orthodontic treatment may be the right decisions that a dental practitioner may consider during the management of such cases.

The mammalian dentition is a serially homogeneous structure that exhibits wide numerical and morphological variation among multiple different species. Patterning of the dentition is achieved through complex reiterative molecular signaling interactions that occur throughout the process of odontogenesis. The secreted signaling molecule Sonic hedgehog (Shh) plays a key role in this process, and the Shh coreceptor growth arrest-specific 1 (Gas1) is expressed in odontogenic mesenchyme and epithelium during multiple stages of tooth development. We show that mice engineered with Gas1 loss-of-function mutation have variation in number, morphology, and size of teeth within their molar dentition. Specifically, supernumerary teeth with variable morphology are present mesial to the first molar with high penetrance, while molar teeth are characterized by the presence of both additional and absent cusps, combined with reduced dimensions and exacerbated by the presence of a supernumerary tooth. We demonstrate that the supernumerary tooth in Gas1 mutant mice arises through proliferation and survival of vestigial tooth germs and that Gas1 function in cranial neural crest cells is essential for the regulation of tooth number, acting to restrict Wnt and downstream FGF signaling in odontogenic epithelium through facilitation of Shh signal transduction. Moreover, regulation of tooth number is independent of the additional Hedgehog coreceptors Cdon and Boc, which are also expressed in multiple regions of the developing tooth germ. Interestingly, further reduction of Hedgehog pathway activity in Shhtm6Amc hypomorphic mice leads to fusion of the molar field and reduced prevalence of supernumerary teeth in a Gas1 mutant background. Finally, we demonstrate defective coronal morphology and reduced coronal dimensions in the molar dentition of human subjects identified with pathogenic mutations in GAS1 and SHH/GAS1, suggesting that regulation of Hedgehog signaling through GAS1 is also essential for normal patterning of the human dentition.

Summary Background/Aim: The study aimed to evaluate the prevalence and distribution of congenital dental number anomalies in the permanent dentition among a group of Turkish children in the Inner Aegean Region of Turkey. Material and Methods: A total of 5377 patients aged 7–9 who visited our clinic for the first time between September 2018 and September 2019 were investigated. The children were examined for tooth agenesis and supernumerary teeth using panoramic radiographs taken for various reasons and clinical records reviewed. Dental number anomalies were evaluated according to gender and localization. Descriptive and comparative statistical analyses were performed using the SPSS package software program Version 23. Results: For the study, the data of 1987 patients (947 female, 1040 male) were examined. The tooth agenesis was found in 109 patients (5.5%), the supernumerary teeth were found in 24 patients (1.2%). The prevalence of tooth agenesis was 5% in males, 6% in females. There was no statistical difference between genders (p>0.05). The distribution of tooth agenesis according to jaws and sides by gender was not statistically different (p>0.05). The most commonly missing teeth were mandibular left second premolar (37.6% of patients) and mandibular right second premolar (33% of patients). According to types of teeth and gender, molar tooth agenesis was seen more common in females than males (p<0.05), there was no significant difference in other types of teeth by gender. The prevalence of supernumerary teeth were 1.9% in males, 0.4% in females and the difference between genders was found to be statistically significant (p<0.05). All the supernumerary teeth were located in the anterior maxilla, and half of them were mesiodens. Conclusions: Early diagnosis and appropriate treatment choice are very important managing complications associated with congenital dental number anomalies and for differential diagnosis of characteristic syndromes.
