Summary Tetralogy of Fallot is the most common cyanogenic congenital heart defect. The diagnosis is based on clinical signs, ECG examination, ultrasound examination of the heart, additional imaging methods and invasive testing. The therapeutic approach to the patient with tetralogy is complex and based on conservative and radical methods. Patients who have not undergone a radical surgical intervention have a poor prognosis, whereas the prognosis is much better for patients who have been operated. The most common complication of the surgical treatment is the pulmonary valve insufficiency which usually requires reintervention, as was the case with our patient.

AbstractDespite tremendous advances in surgical treatment of tetralogy of Fallot, augmenting the small right ventricular outflow tract remains a challenge. Transannular patch augmentation revolutionised surgical management, but did so at the expense of rendering patients with pulmonary insufficiency and the resulting problems associated therewith. Recent surgical efforts have focused on pulmonary valve preservation at initial correction and pulmonary valve restoration after transannular patching, with favourable results. In this manuscript, we review methods of pulmonary valve preservation and restoration.

A newborn infant with tetralogy of Fallot and absent pulmonary valve was successfully corrected in two stages. Absent pulmonary valve syndrome presenting in early infancy manifests severe respiratory symptoms that still make challenging both management and surgical treatment. This is ascribed to tracheobronchial compression by the extremely dilated pulmonary arteries, and to the resultant pulmonary obstructive lesions. We report herein the first findings of 99m Technegas ventilation lung scanning in an infant with the syndrome to assess the pulmonary obstructive lesions.
