Background: Nevus spilus is characterized by a pigmented patch with scattered flat or maculopapular speckles. Nevus spilus was first described by Burkley in 1842. Since then, this lesion has been widely debated in the literature, particularly for the possible occurrence of melanoma within the lesion. Objective: We describe the case of a 65-year-old female presenting with a nodular achromic melanoma that occurred within a nevus spilus on the left thigh. Conclusion: Our observation is consistent with the idea that this entity in some circumstances may have the ability to evolve into a malignant melanoma.

We report a case of necrotizing fasciitis in an 84 year-old man affected by diabetes mellitus. The patient was admitted in the Emergency Department of our hospital because of an acute and strong left leg pain that began almost 8 hours before admission. The left leg had an increased size and a movement limitation, with a hard haemathoma in the left thigh with subcutaneous crepitus. The lesion became worse and larger rapidly, with a wide extension from the back to the popliteal fossa. An antimicrobial therapy was immediately started with morphine for pain. A surgical debridment was performed, but the patient died for multiorgan failure. Necrotizing fasciitis is a rare and mortal disease, the early diagnosis is a challenge for the Emergency Department where patients are admitted and assessed primarly.

Stabbing is the most common method of homicide in developing countries like Bangladesh, due to poverty, over population, joblessness, political unrest, etc. The reported case is of a 27 years old man stabbed on the front of the left thigh, a relatively unusual site by a sharp pointed knife by some snatchers while sitting on a rickshaw. On medicolegal point of view the injury was homicidal in nature to run away without having a motive to kill the person. The site was easily approachable for the snatchers.
