A sixty-six year old lady was admitted after describing two ‘vacant’ episodes with collapse. She had also complained of chronic fronto-occipital headache and more than 10kg weight loss over the preceding three month period. She denied any gastrointestinal, respiratory or cardiac symptoms. She was a non-smoker and did not drink alcohol regularly. She had a past history of hypertension and hypothyroidism for which she was taking ramipril and thyroxine. On examination she appeared cachexic, but no other abnormality was detected.

Tuberculosis may affect many of the endocrine glands including the hypothalamus, pituitary, thyroid with adrenal gland being the commonest. We describe a patient presented with recurrent spontaneous hypoglycemia, hypotension, fever, weight loss and cough. Along with this clinical picture, high ACTH in the face of low cortisol was compatible with diagnosis of Addison’s disease. Rapid ACTH stimulation test affirmed the diagnosis. Disseminated TB affecting adrenal glands was supported by CXR, USG, CT Abdomen and FNAC. Adrenal Crisis was led by add-on bacterial pneumonia. Patient made good recovery with treatment for adrenal crisis and subsequently with standard antitubercular regimen and steroid replacement therapy. Tuberculosis, although uncommon but potentially devastating cause of adrenal failure, merits consideration when fever, weight loss, gastrointestinal symptoms, hyponatremia, hyperkalemia are observed in patients with features of active tuberculosis or past history of tuberculosisJ Bangladesh Coll Phys Surg 2015; 33(1): 44-47

A young male was admitted with diagnosis of Covid-19 SARS infection and was having fever, cough & non-cardiac chest pain. There was no past history of cardiac symptoms and physical examination was unremarkable. His vitals were stable. His ECG showed ST elevation and Tall but notched T wave in V3 and ST depression in inferior and lateral leads. Carefully looking these manifestations, they seem the artifacts based on their ECG features and history of the patient. ECG was repeated removing all possible sources of technical errors for these ECG manifestations which showed normal ECG confirming the diagnosis of artifactual ECG. Patient was discharged uneventfully. Recognition of ECG artifact and their technical causes is necessary to avoid inappropriate management.

Introduction: Abdominal TB is one of the most prevalent forms of extra-pulmonary disease and is prevalent all over the world.Gastrointestinal involvement had been reported to be 55-90%. This study aimed to determine the outcome of stricturoplasty in patients withintestinal tuberculosis. Objective: To determine the outcome of stricturoplasty in patients with intestinal tuberculosis. Study Design:Descriptive case series. Setting: Department of General Surgery, Nishtar Hospital Multan. Duration with dates: Three years from January2009 to December 2011. Subjects & Methods: 120 patients of intestinal TB, who presented with intestinal obstruction in emergency and withsigns and symptoms of intestinal TB in outdoor patient department were selected. History of contact, family history of tuberculosis,immunization, past history of tuberculosis was taken. Diagnosis was confirmed by histopathology. Laparatomy was done in all cases andstricturoplasty was performed. Patients were followed up during hospitalization. Postoperative complications were noted. All information wererecorded on a proforma. Results: Out of 120 patients, 56.7% were male and 43.3% were female having age between 14 to 70 years. Mainpresenting symptoms were pain abdomen, vomiting, distension of abdomen, weight loss, anorexia and low grade fever with sweats. Maincomplications after stricturoplasty were chest infection 40(33.3%), wound dehiscence in 16(13.3%), leakage in 12(10%), recurrence in20(16.7%), scar pigmentation in 12(10%), keloid in 12(10%) patients and herniation in 8(6.7%) patients. Conclusions: Stricturoplasty is a safe,simple and easy operation, particularly useful at small peripheral hospitals with limited staff and resources.

An 86 year old woman with a background of hypertension and arthritis presented to our medical assessment unit with 3 weeks history of mild dyspnoea and central chest discomfort. She did not have any past history of respiratory problems. She denied any cough, hemoptysis or weight loss. There was no significant family history. She was a life long non smoker and used to drink alcohol occasionally. She was taking enalapril 5 mg and amlodipine 5 mg daily. She was living alone and was independent and self caring.

31 year female patient presented to surgical OPD to take consult about incidental finding of large mass originating th from liver in routine antennal checkup at 5 month of pregnancy. We have done ultrasound which is suggestive of suprarenal mass patient was kept under regular follow up and checked for any increase in size of mass but there is no changes in mass .patient deliver baby with normal vaginal delivery ,after postpartum 7 months patient came back with right sided abdominal lump. No complaint of abdominal pain ,vomiting ,headache ,palpitation ,diarrhea ,constipation ,fever ,generalized weakness and weight loss. Past history of cesarean section before 8 years .

Summary Ten years after the successful withdrawal from heroin abuse, a person with diabetes suffered intractable pain and severe muscular emaciation consistent with the syndrome of diabetic neuropathic cachexia. Anti-neuropathic medications failed neither to alleviate suffering and reverse weight loss, nor to stop muscular emaciation. Vigilant evaluation for weight loss aetiologies revealed no responsible aetiology. Prescribing medical cannabis became mandatory, with the intention to alleviate neuropathic pain, regain muscular mass and strengthen legs, enable standing upright and walking normally. Medical cannabis for pain-relief, and the orexigenic properties of tetrahydrocannabinol (THC) ingredient successfully achieved these goals. Learning points: Medical cannabis can serve to promptly alleviate severe diabetic neuropathic pain. Past history of heroin abuse was not an absolute contraindication to medical cannabis use. Medical cannabis increased appetite and reversed muscular emaciation. Medical cannabis decreased chronic pain and hence, its catabolic consequences.

Background and Objective: HIV infection is also a cause of postpartum depression, however, in Vietnam, there has not yet the prevalence of postpartum depression in HIV infected women. The objective is to determine prevalence and related factors of postpartum depression in HIV infected women. Materials and Methods: From November 30th, 2012 to March 30th, 2014, a prospective cohort study is done at Dong Nai and Binh Duong province. The sample includes135 HIV infected women and 405 non infected women (ratio 1/3) who accepted to participate to the research. We used “Edinburgh Postnatal Depression Scale (EPDS) as a screening test when women hospitalized for delivery and 1 week, 6weeks postpartum. Mother who score EPDS ≥ 13 are likely to be suffering from depression. We exclude women who have EPDS ≥ 13 since just hospitalize. Data are collected by a structural questionaire. Results: At 6 weeks postpartum, prevalence of depression in HIV infected women is 61%, in the HIV non infected women is 8.7% (p < 0.001). There are statistical significant differences (p<0.05) between two groups for some factors: education, profession, income, past history of depression, child’s health, breast feeding. Logistical regression analysis determine these factors are related with depression: late diagnosis of HIV infection, child infected of HIV, feeling guilty of HIV infected and feeling guilty with their family. Multivariate regression analysis showed 4 factors are related with depression: HIV infection, living in the province, child’s health, past history of depression. Conclusion: Prevalence of postpartum depression in HIV infected women is 61.2%; risk of depression of postnatal HIV infected women is 6.4 times the risk of postnatal HIV non infected women, RR=6.4 (95% CI:4.3 – 9.4). Domestic women have lower risk than immigrant women from other province, RR=0.72 (95% CI:0.5 – 0.9). Past history of depression is a risk factor with RR=1.7 (95% CI:1.02 – 0.9. A postulate of locality permeates through the special and general theories of relativity. First, Lorentz invariance is extended in a pointwise manner to actual, namely, accelerated observers in Minkowski spacetime. This hypothesis of locality is then employed crucially in Einstein’s local principle of equivalence to render observers pointwise inertial in a gravitational field. Field measurements are intrinsically nonlocal, however. To go beyond the locality postulate in Minkowski spacetime, the past history of the accelerated observer must be taken into account in accordance with the Bohr-Rosenfeld principle. The observer in general carries the memory of its past acceleration. The deep connection between inertia and gravitation suggests that gravity could be nonlocal as well and in nonlocal gravity the fading gravitational memory of past events must then be taken into account. Along this line of thought, a classical nonlocal generalization of Einstein’s theory of gravitation has recently been developed. In this nonlocal gravity (NLG) theory, the gravitational field is local, but satisfies a partial integro-differential field equation. A significant observational consequence of this theory is that the nonlocal aspect of gravity appears to simulate dark matter. The implications of NLG are explored in this book for gravitational lensing, gravitational radiation, the gravitational physics of the Solar System and the internal dynamics of nearby galaxies as well as clusters of galaxies. This approach is extended to nonlocal Newtonian cosmology, where the attraction of gravity fades with the expansion of the universe. Thus far only some of the consequences of NLG have been compared with observation.
