Introduction: Varicella zoster virus (VZV) meningitis is primarily an infection of the immuno-compromised. However, it can also affect immunocompetent individuals. Reactivation of VZV typically presents with a distinct dermatomal rash suggestive of varicella zoster, but there have also been reports of VZV meningitis presenting without a rash. Case Report: We describe a case of VZV meningitis in a healthy, 30-year-old male presenting to the emergency department shortly after receiving his first coronavirus disease 2019 vaccination. He was treated with intravenous acyclovir and then discharged home on oral valacyclovir. Conclusion: Emergency physicians should consider aseptic meningitis in immunocompetent patients presenting with atypical headaches in this population.

Abstract Background Varicella-zoster virus (VZV) is a known cause of aseptic meningitis, with a predisposition for an immunocompromised population. A dermatomal rash usually accompanies aseptic meningitis secondary to VZV. Case presentation We report the case of a 31-year-old male with a history of chickenpox in childhood and recent shingles who presented with severe frontal headaches secondary to VZV meningitis. The patient had also recently received the measles-mumps-rubella (MMR) vaccine. He recovered without any neurological sequala. Conclusion This case report describes an immunocompetent patient with recent MMR vaccination who developed aseptic meningitis secondary to VZV without any dermatomal involvement (Zoster Sine Herpete).

Abstract Background Meningitis retention syndrome (MRS) is a rare condition that presents with acute urinary retention as a complication of aseptic meningitis. Cases of MRS due to varicella zoster virus (VZV) infection without a rash are rare. We report the case of a patient who had no signs of meningitis or VZV infection, including a rash. Case presentation A 58-year-old man presented with dysesthesia of the lower limbs and acute urinary retention. He had fever but no rash and no signs of meningitis. He was diagnosed to have VZV infection based on the detection of VZV DNA in the cerebrospinal fluid. He responded satisfactorily to a course of intravenous acyclovir and experienced no sequelae during a 2-year follow-up period. Conclusion MRS due to aseptic meningitis of viral origin should be considered in the differential diagnosis of acute urinary retention even in the absence of specific signs and symptoms of meningitis or a suggestive rash.

Herpes zoster or shingles is caused by the reactivation of latent varicella zoster virus (VZV) infection within the sensory ganglia. Primary VZV infection results in varicella (chickenpox) usually in childhood, characterized by vesicular lesions on the face, trunk, and extremities. Although herpes zoster can occur at any age, it is mainly a disease of adults over age 50 and/or immunocompromised individuals. Complications of herpes zoster include post herpetic neuralgia, herpes zoster ophthalmic us and less commonly acute retinal necrosis, aseptic meningitis, and encephalitis. Here we present the case of a 51-year-old female patient with Herpes Zoster infection involving the dermatome on the forehead, initially misdiagnosed, to alert clinicians to create awareness and minimize misdiagnosis of other patients with similar cases. It is unusual to find patients presenting with infection involving the dermatome of the forehead.

Dysrhythmias are commonly encountered in the Emergency Department (ED). Timely management and recognition of dysrhythmia in the ED is essential and crucial. Physicians working in the ED should be familiar with atypical presentation of common dysrhythmias. This case report illustrates atrioventricular nodal re-entrant tachycardia (AVNRT) with an unusually slow heart rate in an adolescent. A 16-year-old male presented to the ED twice for palpitations. His electrocardiograms showed narrow complex tachycardia with absent P waves. Electrophysiology studies confirmed typical slow-fast AVNRT which was treated successfully with radiofrequency ablation of the slow pathway. Diagnosing rare dysrhythmias or common dysrhythmias which manifest atypically can be challenging for non-cardiologists. Although AVNRT generally occurs with a heart rate of >130 bpm, emergency physicians should be aware that AVNRT can take place with a relatively slower rate in both the elderly and young.

Herpes zoster (HZ), caused by the varicella-zoster virus, is an infectious skin disease that rarely recurs after initial presentation. The mechanism underlying HZ recurrence is currently under investigation. In this article, we report a case of HZ relapse within 1 month. Analysis of patient’s clinical manifestations, histopathological features, and flow cytometry results indicated that the absolute and percentage values of B cells were below the lower limit. We hypothesized that the patient had abnormal humoral immune function, which may be one reason leading to the HZ relapse within 1 month. The findings of this case will serve as useful reference for HZ recurrence for clinicians. This case was impactful and added to the literature on HZ recurrence.
