Two infants with congestive failure in the newborn period were found to have congenital arteriovenous fistulae at post-mortem examination. One case died at 5 months of age with advanced neurologic disease; the other died at 3 days in severe congestive heart failure. In neither case were there developmental cardiac anomalies. A brief review of the literature on congenital cerebral arteriovenous aneurysms and a discussion of their circulatory effect is presented. It is suggested that the neonatal heart works near its maximum capacity and congestive failure may be readily precipitated. Extracardiac conditions responsible for congestive failure are enumerated.

Between 1984 and 1988, a resurgence of acute rheumatic fever (ARF) was reported in several pediatric centers.1-3 During the same time, pediatric cases,4-6 and, recently, adult cases6 of poststreptococcal reactive arthritis (PSRA) have been reported. Although several of these patients had or developed multisystemic manifestations,4,5 none of these cases fully adhered to the guidelines of the revised Jones criteria.7 Thus, these individuals were given the diagnosis of PSRA or poststreptococcal rheumatic syndrome (PSRS) rather than ARF.4-6,8-10 One of the leading causes of acquired pediatric heart disease in North America is carditis of ARF,3 and severe cases can result in congestive heart failure, the need for valve replacement, or death.11

Exercise and cardiac rehabilitation have been underused therapy options for patients with congestive heart failure despite being recommended in international guidelines and being covered by Medicare in the US. This article reviews the evidence behind this treatment strategy and details current trials that will contribute to the evidence base.
