Chronic lymphatic leukemia is a lymphoproliferative disease typical in adult age. It is characterized by a proliferation in the peripheral blood and bone marrow of mature lymphocytes progressively permeating the lymphonodes, spleen, liver and more rarely the uro-genital apparatus. This case report concerns a man suffering from C.L.L. since 1972, treated with medical therapy; from 1991 onwards the patient reported a pain in the right lumbar area. Radiological examinations showed a subcapsular corpuscular mass 3.3 cm thick with no change in the kidney parenchyma. Examination of the blood showed hyperleucocytosis. The patient underwent right nephrectomy after biopsy; the histological examination showed leukemic infiltration compatible with C.L.L. The patient then underwent therapy with Fludarabine and is at present in good condition.

Chronic lymphatic leukemia is second only to acute leukemia in the frequency of infectious complications. In most cases, severe infectious complications are the cause of death in these patients. Modern chemotherapy makes it possible to preserve the ability to work and the life expectancy of patients for a long time. However, the negative effect of cytostatic drugs on the already altered immune system leads to an even greater suppression of immunity and an increase in the number of infectious complications. The search for new ways to combat infections has shown the advisability of long-term outpatient treatment with antibacterial drugs.
