The purpose of this work is to evaluate the impacts of body off-center positioning on CT numbers and dose index CTDIv of two scanners from GE. HD750 and APEX scanners were used to acquire a PBU60 phantom of Kagaku and a 062M phantom of CIRS respectively. CT images were acquired at various off-center positions under automatic tube current modulation using various peak voltages. CTDIv were recorded for each of the acquisitions. An abdomen section of the PBU60 phantom was used for CT number analysis and tissue inserts of the 062M phantom were filled with water balloons to mimic the human abdomen. CT numbers of central regions of interests were averaged using the Fiji software. As phantoms were lifted above the iso-center, both CTDIv and CT numbers were increased for the HD750 scanner whilst they were approximately constant for the APEX scanner. The measured sizes of anterior-posterior projection images were also increased for both scanners whilst the sizes of lateral projection images were increased for the HD750 scanner but decreased for the APEX scanner. Off-center correction algorithms were implemented in the APEX scanner. Matching the X-ray projection center with the system’s iso-center could improve the accuracy of CT imaging.

AbstractOn March 11, 2020, the World Health Organization declared the coronavirus disease 2019 (COVID-19) pandemic. The expert organisations recommend more cautious use of thoracic computed tomography (CT), opting for low-dose protocols. We aimed at determining a threshold value of automatic tube current modulation noise index below which there is a chance to miss an onset of ground-glass opacities (GGO) in COVID-19. A team of radiologists and medical physicists performed 25 phantom CT studies using different automatic tube current modulation settings (SUREExposure3D technology). We then conducted a retrospective evaluation of the chest CT images from 22 patients with COVID-19 and calculated the density difference between the GGO and unaffected tissue. Finally, the results were matched to the phantom study results to determine the minimum noise index threshold value. The minimum density difference at the onset of COVID-19 was 252 HU (p < 0.001). This was found to correspond to the SUREExposure 3D noise index of 36. We established the noise index threshold of 36 for the Canon scanner without iterative reconstructions, allowing for a decrease in the dose-length product by 80%. The proposed protocol needs to be validated in a prospective study.

In the screening and identifying of colon and rectum malignancy, computed tomography colonography (CTC) is a highly effective imaging technique, albeit patients receiving a significant effective dose. Accordingly, patient dose evaluation is an important need, seeking to ensure benefits outweigh the projected cancer risk. Objective: For CTC procedures carried out in the Radiology Department, Medical Imaging Operation Services, King Fahad Medical City (KFMC), evaluation is done using the current American College of Radiology (ACR) imaging protocol and concomitant patient-effective doses. Study is carried out on a sample size of 55 CTC procedures, involving 25 males (45%) and 30 females (55%). The patients were classified as follows: two groups based on CT machine; four groups based on the applied protocol; and three groups based on the procedure results. All procedures were carried out using two machines, the products of two different vendors (a GE Healthcare DISCOVERY CT 750 HD 64 slices dual-energy scanner and a Philips Brilliance CT 64 slices scanner). The overall mean, standard deviation (SD), median, and range of the effective dose (in mSv) were 11.57 ± 7.75, 9.25 (2.17–31.93). Automatic tube current modulation (ATCM) shows a significant increase in CTDIvol up to 69% and effective dose (mSv) up to 95% than the manual tube current (mA) compared to the standard protocol. The CT protocol variation results in a three-fold variation in patient-effective dose. The technologist role is crucial in selecting a noise reference based on patient weight and adjusting tube current per slice to avoid overexposure during ATCM protocol.
