BackgroundChina set out the vision to establishing a hierarchical medical system, with primary health care (PHC) facilities serving health care users’ first contact. Common ailments were listed, supported by a series of auxiliary policy measures. We aim to assess whether these policies were effective to prompt users’ preference to PHCs within these contexts.MethodsUsing data from three waves of National Health Service Survey, we examined trends in care users’ first contact with PHC facilities in Jilin, a north eastern province, during 2008–2018. We analysed trends and factors affecting care users’ choices, stratified by type of diseases and urban–rural settings.ResultsFrom 38 823 respondents, the survey identified 3302 health care users who sought outpatient care. 54.92% and 82.49% with diseases recommended to PHC, in urban and rural Jilin, respectively, contacted PHC facilities first. While 33.51% and 61.19% with diseases not recommended to PHC did so. Care users’ first contact with PHC facilities followed an inverse U shape during 2008–2018. Such trends were more profound among care users with hypertension and/or diabetes. Neither social health insurance coverage nor contracting with family doctors was associated with care users’ first contacts. Only 1.25% care users had referral experiences. Low perceived quality was the main barrier to choose PHC facilities.ConclusionHealth care users sought PHC in a chaotic manner in Jilin. None of the recent efforts seemed effective in prompting their preference to PHC facilities. Without levering quality of PHC, an effective hierarchical medical system could be hardly forged in China.

The Jinchang gold–copper deposit is located in Eastern Heilongjiang Province,Northeastern China. The orebody comprises primarily hydrothermal breccias, quartz veins, anddisseminated ores within granite, diorite, and granodiorite. Three paragenetic stages are identified:early quartz–pyrite–arsenopyrite (Stage 1), quartz–pyrite–chalcopyrite (Stage 2), and latequartz–pyrite–galena–sphalerite (Stage 3). Gold was deposited during all three stages and Stage 1was the major gold-producing stage. Copper is associated with the mineralization but has loweconomic value. Fluid inclusions (FIs) within the deposit are liquid-rich aqueous, vapor-rich aqueous,and daughter-mineral-bearing types. Microthermometric data for the FIs reveal decreasinghomogenization temperatures (Th) and salinities of the ore-forming fluids over time. The Th forStages 1–3 of the mineralization are 421–479, 363–408, and 296–347 °C, respectively. Stage 1 fluidsin vapor-rich and daughter-mineral-bearing inclusions have salinities of 5.7–8.7 and 49.8–54.4 wt%NaCl equivalent, respectively. Stage 2 fluids in vapor-rich, liquid-rich, and daughter-mineral-bearinginclusions have salinities of 1.2–5.4, 9.5–16.0, and 43.3–48.3 wt% NaCl, respectively. Stage 3 fluids inliquid-rich and daughter-mineral-bearing inclusions have salinities of 7.9–12.6 and 38.3–42.0 wt% NaClequivalent, respectively. The estimated trapping pressures are 160–220 bar, corresponding toan entrapment depth of 1.6–1.2 km in the paleo-water table. Oxygen and hydrogen isotope data(δ18OV-SMOW = 8.6‰ to 11.4‰; δDV-SMOW = −92.2‰ to −72.1‰) suggest that the ore-forming fluidswere derived from magmatic fluids during the early stages of mineralization and subsequentlyincorporated meteoric water during the late stages. The sulfide minerals have δ34SVCDT values of0.2‰–3.5‰, suggesting that the sulfur has a magmatic origin. The Jinchang deposit is a typicalgold-rich gold–copper porphyry deposit.

This paper does unidirectional fluid-solid coupling calculation on the runner strength under three designed head loading conditions of a certain Francis turbine in the north-eastern China. The water pressure on the blade in the flow fields of different operating conditions is calculated by means of CFD software CFX. With the help of ansys workbench, the water pressure is loaded to the blade as structural load to conclude the static stress distribution and deformation of the runner under different operating conditions. The results show that the maximum static stress increases with the rise of the flow and appears near the influent side of the blade connected to the runner crown; the maximum deformation increases with the rise of the flow and appears on the band. The results provides effective basis for the structural design and safe operation of the Francis turbine.
