Getting adequate sleep is a healthy lifestyle activity that is important for the body to function optimally both physically and mentally. Unfortunately, 70 million Americans suffer from chronic sleep loss or sleep disorders. Secondary causes of sleep disorders are often overlooked and can occur from the consumption of social or medicinal drugs. Common social drugs that can disrupt sleep include alcohol, caffeine, and nicotine. Certain medications can also have side-effects that can induce sleep disruptions. This article briefly reviews common social drugs and prescription medications that can negatively impact sleep.

This chapter reviews the architecture and functions of sleep, how to interview patients on their sleep histories, how to evaluate for sleep disorders, commonly encountered sleep disorders and their pathophysiology, and the conventional and integrative therapies for each. The evaluation process includes a conventional sleep study as well as addressing vitamin D, B, and magnesium status. Sleep disorders discussed in this chapter are obstructive sleep apnea, insomnia, restless legs syndrome, periodic limb movement disorder, rapid-eye-movement behavior disorder, circadian rhythm disorders, and narcolepsy and other central hypersomnias. The role of conventional tools and times when alternative and complementary therapies may be considered are discussed in detail. Treatment covered include continuous positive airway pressure, oral appliance, myofunctional therapy, cognitive-behavioral therapy for insomnia, physical modalities, acupuncture, light therapy, melatonin, nutraceuticals, and other supplements that aid with sleep and daytime symptoms.

Sleep disturbances and disorders are common during pregnancy, and they can be risk factors for a number of serious pregnancy-related sleep disorders. These include postpartum depression, pregnancy-induced hypertension, gestational diabetes, and intrauterine growth retardation. In addition, certain chronic sleep disorders, such as narcolepsy, are quite challenging to manage in the context of pregnancy and during lactation, particularly with medications that can be teratogenic. This review discusses 4 common sleep disorders and their impacts on pregnancy, and suggests ways to deal with these disorders that improves maternal and fetal outcomes safely. The review discusses diagnosis and treatment of obstructive sleep apnea, restless legs syndrome, insomnia, and narcolepsy in pregnancy.
