In this chapter, the authors describe a new framework for pervasive healthcare applications where the patient’s consent has a pivotal role. In their framework, patients are able to control the disclosure of their medical data. The patient’s consent is implicitly captured by the context in which his or her medical data is being accessed. Context is expressed in terms of workflows. The execution of a task in a workflow carries information that the system uses for providing access rights accordingly to the patient’s consent. Ultimately, the patient is in charge of withdrawing consent if necessary. Moreover, the use of workflow enables the enforcement of the need-to-kwon principle. This means that a subject is authorised to access sensitive data only when required by the actual situation.

The collection of medical data for research purposes is a challenging and long-lasting process. In an effort to accelerate and facilitate this process we propose a new framework for dynamic aggregation of medical data from distributed sources. We use agent-based coordination between medical and research institutions. Our system employs principles of peer-to-peer network organization and coordination models to search over already constructed distributed databases and to identify the potential contributors when a new database has to be built. Our framework takes into account both the requirements of a research study and current data availability. This leads to better definition of database characteristics such as schema, content, and privacy parameters. We show that this approach enables a more efficient way to collect data for medical research.

A pilot medical information system is being implemented and currently is providing services for limited categories of patient data. In one year, physicians’ diagnoses for 500,000 office visits, 300,000 drug prescriptions for outpatients, one million clinical laboratory tests, and 60,000 multiphasic screening examinations are being stored in and retrieved from integrated, direct access, patient computer medical records.This medical information system is a part of a long-term research and development program. Its major objective is the development of a multifacility computer-based system which will support eventually the medical data requirements of a population of one million persons and one thousand physicians. The strategy employed provides for modular development. The central system, the computer-stored medical records which are therein maintained, and a satellite pilot medical data system in one medical facility are described.
