Perioperative care of the neurosurgical patient describes the processes required to facilitate safely and professionally neurological operative interventions and outcomes. An account is given of the systematic preoperative evaluation needed to optimize a patient’s medical conditions before surgery, the intraoperative anaesthetic management, the postoperative care, and the possible complications that may be encountered. Anaesthetic considerations for specific neurosurgical interventions are also described. Overall, emphasis is placed on the importance of requesting the appropriate investigations, adhering to strategies for the correction of coagulation disorders along with the prevention of venous thromboembolic complications, understanding the effect of anaesthesia on cerebrovascular haemodynamics and providing specialist postoperative facilities and expertise.

AbstractThe subciliary lower eyelid blepharoplasty has evolved considerably to create a more harmonious natural appearance with a fuller and unoperated look and also to minimize the complications. While lower eyelid malposition was very common in the past, now this complication is significantly reduced by attention to preoperative evaluation, meticulous surgical planning, precise surgical technique, and postoperative care. Various prophylactic maneuvers maintaining/strengthening lower lid support can be utilized to prevent lower lid malposition including preservation of the pretarsal orbicularis oculi muscle, conservative resection of skin and muscle, and suspension of the orbicularis oculi muscle and/or tarsus to the periosteum of the lateral orbital rim. The release of the orbicularis retaining ligament and surgical transposition of orbital fat over the rim rather than excision allows for smoothing of the lid-cheek junction, filling the tear trough deformity, and reducing the appearance of bulging fat in the lower eyelid. In this article the reader will find a comprehensive approach for achieving a smooth contour with gradual blending at the lower eyelid–cheek junction while maintaining/restoring normal lower lid support. A descriptive outline of postoperative care is also provided to help in optimal healing for the patient.

In this chapter the essential aspects of anesthesia for repair of tetralogy of Fallot (TOF) are reviewed. Subtopics include hemodynamic considerations in TOF patients such as right ventricular hypertrophy, components of TOF, and circulation in the TOF patient. The chapter is divided into preoperative, intraoperative, and postoperative sections with important subtopics related to the main topic in each section. Issues discussed that are related to preoperative evaluation include differentiating between cyanotic and acyanotic cardiac lesions, recognizing Tet spells and systemic vascular resistance, and use of antibiotic prophylaxis. Intraoperative topics include induction, onset time of anesthetic agents, and blood therapy. Postoperative care addressed includes extubation, hypothermia, and hypoxia.

The authors present a retrospective study evaluating the results of calcaneonavicular bar resection with interposition of the extensor digitorum brevis in ten patients with painful calcaneonavicular coalition. All operative procedures were supervised by one of the authors to ensure consistency in preoperative evaluation, surgical technique, and postoperative care. Results obtained from questionnaires showed significant improvement of preoperative symptoms with increased activity levels noted in all patients.

Upper and lower blepharoplasties are some of the most common cosmetic surgical procedures. The eyelids function in ocular protection and lubrication and are capable of expressing a wide range of emotions in nonverbal communication. As such, the goals of surgery are to impart a natural rejuvenated appearance with adequate symmetry while preserving the normal function of the eyelids. Patients seeking blepharoplasty can present with any number of chief complaints that can be categorized as functional or aesthetic in nature. The two are, however, intimately intertwined and the surgeon must consider both the function and appearance of the eyelids. Herein, we discuss various approaches to the clinical management of patients for blepharoplasty, including preoperative evaluation, operative approaches, and postoperative care.
