We describe a case of osteoid osteoma in the radial styloid of a 37-year old man mimicking de Quervain's disease. The exact diagnosis was made with CT examination and confirmed by histological evaluation of the tumour. The lesion was removed by curettage and symptoms were resolved rapidly thereafter. There was no sign of recurrence at one year follow-up.

A clinical test for scaphoid waist fractures is described in which the first ray is compressed longitudinally. This has the advantage that it can be done when the patient is in a plaster cast. Of 52 patients with suspected scaphoid fractures, 37 had negative tests and proved by X-rays then and later to have no fracture. 12 had positive tests and X-rays. Two patients with positive tests had fractures nearby: one of the scaphoid tuberosity and one of the radial styloid. One patient with a negative test had de Quervain’s disease. Once union has occurred, the test becomes negative.

The basic anatomy of the first extensor compartment is presented with a review of the pathology of de Quervain’s stenosing tenovaginitis. The results in 100 medicolegal cases of de Quervain’s disease are analysed and reasons are sought for the poor results. A review of the known factors associated with the causation of de Quervain’s disease is presented with recommendations about the management of this condition.

AbstractPersistent pain despite previous surgery for de Quervain’s disease might be due to an overlooked septum between the abductor pollicis longus tendon slips and the extensor pollicis brevis tendon, or an overlooked completely separate compartment for the extensor pollicis brevis tendon alone. In both of these instances, extension of the MP joint of the thumb against resistance elicits pain at the distal level of the first extensor compartment of the wrist. When this sign is positive, revisional surgery and decompression of the remaining septum or separate compartment is indicated.

The purpose of this study was to establish a normal measure of scaphoid position in the radioulnar plane in standard neutral, radial and ulnar deviation posteroanterior radiographs. This measurement may allow indirect evaluation of the radiocarpal ligaments and comparison between normal and pathologic states (following radius fractures, perilunate dislocations). Measurements were trialed on 74 normal wrist radiographs and 25 cadaver wrists. We evaluated the distance between the radial styloid and the scaphoid and corresponding scaphoid width. The ratio of distance/width at the mid styloid level (0.35, imprecision SD = 0.1) had the lowest random error and is therefore the most precise measurement of true scaphoid translation. This measurement is independent of scapholunate ligament integrity and may provide a better assessment of the radiocarpal component of ulnar translational instability. Abnormal movement of the scaphoid in the radioscaphoid joint likely reflects ligamentous injury. Identifying and addressing these injuries may prevent the development of arthritis.
