Background: In the last years thoracic surgery developed in greater extent with equipments and techniques in one lung ventilation. Still general anesthesia in one lung ventilation approved as gold standard. In thoracic surgery most performed surgeries are plerural decortication and lung biopsy. Avoidance of intubation in Video Assisted Thoracoscopic Surgery (VATS) procedures gains us some advantages in postoperative period; a better respiratory parameters, survival and morbidity mortality rates, reduced hospitalization time and costs, reduced early stress hormone and immune response.  Methods: In this study, we reported our experience of 24 consecutive patients undergoing VATS with Thoracic Epidural Anesthesia (TEA) between December 2015 through July 2016 to evaluate the feasibility, safety and indication of this innovative technique whether it will be a gold standart in thoracic surgeries or not in the future.Results: Operation procedures included wedge resection in 11 (46%) patients (eight of them for pneumothorax, three of them for diagnosis), in 10 (42%) patients pleural biopsy (eight of them used talc pleurodesis), in two (8%) patients air leak control with fibrin glue and in one (4%) patient bilateral thoracal sympathectomy for hyperhidrosis.  We used T4-5 TEA space for 17 (72%) of patients, while we used T4-6 TEA space for 7 (28%) of patients. TEA block reached the desired level after the mean 26.4±4.3 minutes (range 21-34 min). There was no occurrence of hypotension and bradycardia during and after TEA. One (4%) patient required conversion to general anesthesia and tracheal intubation because of significant diaphragmatic contractions and hyperpne. Conversion to thoracotomy was not needed in any patient.Conclusions: We conclude that nVATS procedure with aid of TEA is feasibile and safety with minimal adverse events. The procedure can have such advantages as early mobilization, opening of early oral intake, early discharge, patient satisfaction, low pain level. Nevertheless, there is a need for randomized controlled trials involving wider case series on the subject.

Įvadas / tikslasPleuros empiema – viena iš seniausiai žinomų krūtinės ligų ir iki šiol yra susijusi su didėjančiu sergamumu visame pasaulyje. Kaip ir daugelyje chirurgijos sričių, gydant pūlinėmis pleuros ligomis sergančius ligonius populiarėja minimaliai invazinės chi­rurginės procedūros. Tačiau torakoskopinė operacija vis dar negali pakeisti atvirosios visais empiemų atvejais. Šiame straips­nyje supažindiname su mūsų atliekamų torakoskopinių pleuros empiemos operacijų metodika ir aptariame pradinę patirtį.Ligoniai ir metodaiNuo 2011 m. sausio iki 2013 m. rugsėjo torakoskopiškai buvo operuoti 49 ligoniai, sergantys pleuros empiema. Visi pacientai operuoti sukėlus bendrinę nejautrą, naudojant vieno plaučio ventiliaciją. Visos torakoskopinės operacijos buvo padarytos per dvi arba tris angas krūtinėje. Tais atvejais, kai sėkmingai atlikti torakoskopiniu būdu empiemektomijos nepavykdavo, būdavo pereinama į atvirąją (torakotominę) operaciją.RezultataiSėkmingai videotorakoskopinė operacija buvo atlikta 36 (73,5 %) ligoniams, o 13 (26,5 %) atvejų prireikė konversijos į atvirą operaciją. Pleuros ertmės sąaugos ir negalėjimas iki galo pašalinti ant plaučio paviršiaus susidariusio jį kaustančio šarvo buvo pagrindinės konversijų priežastys. Dešimt (20 %) ligonių patyrė tokių pooperacinių komplikacijų kaip ligos atkrytis, ilgesnį laiką besiskiriantis pro drenus oras, žaizdos infekcija.IšvadosPleuros empiemos atveju minimaliai invazinė chirurgija yra saugi ir pakankamai efektyvi. Abstract Objective: To investigate the short- and medium-term outcomes following treatment with uniportal video-assisted thoracic surgery lobectomy (uniportal VATS) in elderly patients with non-small cell lung cancer (NSCLC). Methods: We conducted a retrospective analysis on the clinical and follow-up data of 74 elderly patients with NSCLC who underwent uniportal VATS between January 2015 and January 2020. One-to-one propensity score matching (PSM) was employed to select 71 elderly patients with NSCLC who underwent multiportal video-assisted thoracoscopic lobectomy (multiportal VATS) during the same period. Results: The baseline characteristics of the two patient groups were comparable, with no statistically significant differences in postoperative complications, operation time, conversion to thoracotomy, or lymph node dissection. The amount of intraoperative blood loss and postoperative pain were lower in the uniportal VATS group than in the multiportal VATS group. The 3-year overall survival and disease-free survival of the two groups were similar. Conclusions: Uniportal VATS achieved similar short- and medium-term outcomes as Multiportal VATS in elderly patients with NSCLC.

Absztrakt: Bevezetés: Közleményünkben ismertetjük a 2013–2019 közötti időszakban osztályunkon minimálisan invazív módszerrel (video-assisted thoracic surgery – VATS) elvégzett tüdőlobectomiák során szerzett tapasztalatainkat, eredményeinket. Betegek és módszer: 2013. november 1. és 2019. június 30. közötti időszakban osztályunkon 112 VATS lobectomiát végeztünk. A műtéteket izolált intubációval altatott betegeken elülső behatolás technikával végeztük. 98 betegnél malignus, 9 betegnél benignus elváltozás miatt történtek a műtétek. 10 esetben a műtét utáni szövettani diagnózis metasztázis volt. 78 férfi és 34 nőbetegen végeztük a műtéteket. A betegek átlagéletkora 60,5 (42–63) év volt. Az eltávolított lebenyek megoszlása a következő volt: bal felső: 5, bal alsó: 36, Jobb felső: 15, középső: 11, jobb alsó: 44, pulmonectomia: 1. Eredmények: Műtéti halálozás nem volt. Konverzióra 3 alkalommal kényszerültünk. Az átlagos műtéti idő 150 (70–215) perc volt. Reoperációt 2 betegnél végeztünk, vérzés, légáteresztés miatt. A 112 műtét során 88 betegnél primer tüdőrák, 9 betegnél jóindulatú elváltozás/gyulladás miatt történt a műtét. 10 esetben más szerv tumora miatti metasztázis miatt végeztük a beavatkozásokat. A primer tüdőrák miatt végzett műtétek stádium szerinti megoszlása a szövettani eredmények alapján a következő volt: I.a: 57, I.b: 22, II.a: 6, II.b: 3. A szövettani típus szerinti megoszlás: adenocarcinoma: 52, laphámrák: 23, kissejtes carcinoma: 2, nagysejtes carcinoma: 5, carcinoid: 6 volt. Következtetés: Osztályunkon a szükséges tanuló időszak után a VATS lobectomia megfelelő indikáció szerint rutin műtéti eljárássá vált. Jelenleg mellkasi műtéteink 65%-a, a lobectomiák több mint 50%-a VATS technikával történik. Eredményeink az országos és az irodalmi adatoknak megfelelnek.
