SummaryHyperextension injury of the tarsometatarsal joint (TMTJ) is a debilitating injury and arthrodesis of this joint is often the only procedure that can result in full return to function. Most surgical procedures described for arthrodesis of the tarsometatarsal joint necessitate the use of external, splint or cast, to protect the implants used from fatigue failure. This report describes the successful use of type II external fixator that spans the tarsometatarsal joint in four dogs with hyperextension injury of the tarsometatarsal joint. This technique enables the patient to bear weight on the limb immediately after surgery until bony fusion is achieved, without the need for an additional protecting splint.

Kessler’s extensor pollicis brevis (EPB) palmar tendon sling is a simple and reliable reconstruction for symptomatic palmar instability of the thumb metacarpophalangeal (MP) joint. However, we encountered subluxation of the extensor pollicis longus tendon and extension lag at the MP joint when the entire tendon was used. We modified the technique, splitting the tendon to preserve its function as an MP joint extensor. Six thumb MP joints with anteroposterior instability secondary to hyperextension injury were reconstructed using the split-EPB technique. At an average of 22 months postoperatively, all patients had stable and pain-free MP joints. Pinch strength improved an average of 5.6 kg. MP joint flexion was decreased an average of 17.5° and two patients had flexion contractures of 5° and 20°, respectively. Extensor pollicis longus subluxation and MP extension lag did not occur, and there were no recurrences.

SummaryA carpometacarpal hyperextension injury in a cat was treated by pancarpal arthrodesis, following the failure of conservative treatment. An 8-hole 1.5 mm mini plate was placed on the dorsal aspect of the distal radius, carpus and third metacarpal bone. An aluminium spoon splint was used for six weeks postoperatively, until radio-graphs revealed bone union. The body weights and the dimensions of the distal radius, radiocarpal bone and third metacarpal bone from the left limb of 15 cadavers were recorded in order to aid the selection of appropriate implants for future cases.Pancarpal arthrodesis was used to treat a cat with a carpometacarpal hyperextension injury. The use of an 8-hole 1.5 mm mini plate applied to the dorsal surface of the carpus, in combination with limb splinting provided effective stabilization. An excellent functional result was obtained. Measurements of the radius, radial carpal and third metacarpal bones from 15 cadavers suggest that 1.5 mm bone screws are the largest that should be used for carpal arthrodesis in the average sized cat.

✓A rare case of epidural hematoma of the clivus is reported in an 11 -year-old girl involved in a traffic accident which caused a severe hyperextension injury. Only one similar case has been reported in the literature. The mechanism for the formation of the hematoma of this region is discussed.
