Le neoplasie squamose papillari delle vie aeree digestive superiori sono una rara variante del carcinoma a cellule squamose. Sono caratterizzate da una crescita esofitica papillare e hanno una prognosi generalmente favorevole. Il tumore è già stato descritto a livello delle vie aeree digestive superiori. In tale contesto, le localizzazioni più frequenti sono la laringe e l’ipofaringe, mentre raramente sono interessati la cavità orale e l’ipofaringe. Gli studi limitati unitamente all’esiguo numero di casi pubblicati di carcinoma squamoso papillare a localizzazione tonsillare, ci hanno indotto a una completa analisi di questo tumore, analizzando gli aspetti clinici, istopatologici, radiologici, virologici e terapeutici, non sempre presenti in letteratura. Un case report di carcinoma squamoso papillare della tonsilla palatina è pertanto riportato. La lesione (T2N0M0), localizzata a livello della tonsilla palatina sinistra, si aggettava verso la cavità orale. HPV DNA 16 e mRNA E6/E7 erano rilevati nella lesione. Un profilo della neoplasia è pertanto presentato unitamente a una completa revisione della recente letteratura, analizzando tutti gli aspetti di interesse di tale neoplasia.

ABSTRACT Tonsilloliths, also known as tonsil stones or tonsillar calculi, are clusters of calcified material that form in the crypts of palatine tonsils. They are usually of small size. Abstract Introduction Achondroplasia is the most common form of dwarfism in humans. At the end of the twentieth century, achondroplasia had its natural history investigated and its morbimortality understood. In dwarves, obesity is one of the causes of morbidity, and it is difficult to evaluate it due to the great disproportion among anthropometric data. The characterization of the nutritional needs of these patients represents an interesting dilemma for physicians. In view of these difficulties, the present case report describes an alternative to obtain the ideal weight value of an elderly dwarf in the use of parenteral nutritional therapy. Case Report A 73-year-old male patient, carrier of achondroplastic dwarfism, was admitted for surgical treatment of colon adenocarcinoma. He was submitted to total colectomy with ileum-rectum anastomosis, evolving with abdominal septic shock by suture dehiscence, which was fixed, and a protective ileostomy was installed. Due to the maintenance of prolonged fasting, without the possibility of starting oral or enteral feeding, the patient used total parenteral nutrition. To estimate the total energy expenditure, we chose to ask the patient what his ideal weight was. The patient used total parenteral nutrition for eight days until he was able to meet his caloric needs orally. Conclusion In the absence of indirect calorimetry, the ideal weight mentioned by the patient seems to be a good option for the estimation of the caloric expenditure by simplified equations.

Background. A case report of a nonhealing ulcer of the tongue histologically proven to be adenocarcinoma.Method. A 92-year-old man underwent clinical, immunochemistry, and imaging investigation.Results. Tests confirmed a distant metastasis of a primary colorectal carcinoma.Conclusion. Metastasis from colorectal carcinoma to the oral cavity is primarily to bone, but non-healing ulcers of the oral cavity should be considered in differential diagnoses.

Introduction S-1, a compounding agent of tegafur, gimeracil, and oteracil potassium, is one of the most effective chemotherapeutic agents for colorectal cancer. In this case, following S-1 administration, we observed predominant elevation of serum triglyceride. Case report A 49-year-old man with stage IV transverse colon adenocarcinoma received S-1 + irinotecan + bevacizumab. At the end of the S-1 administration period in every course, his serum triglyceride level was found to be elevated. Finally, it reached grade 4, without any symptoms of acute pancreatitis in the fifth course, and fenofibrate 80 mg once a day was administered. Management & outcome: Interestingly, the elevation spontaneously normalized without any pharmacotherapy 14 days after S-1 withdrawal, and this elevation did not occur when S-1 was not administered. Further, fenofibrate administration attenuated the hypertriglyceridemia to grades 1-3, with no complications. Discussion S-1 administration induced hypertriglyceridemia owing to the elevated serum triglyceride; however, a contrasting result was observed in the S-1 withdrawal period and during the S-1-cessation cycle. Since dietary intake was poorer during the S-1 administration period, it is considered that S-1 might have disturbed lipid metabolism. Further, we know that capecitabine, which is a prodrug of fluorouracil, also induces hypertriglyceridemia. As the end product of these medicines is fluorouracil, the presence of fluorouracil or its metabolizing enzymes, the genetic background of the patient might have affected the results. We have to be aware of the risk of asymptomatic and temporal occurrence of hypertriglyceridemia by S-1 administration for the early detection with appropriate pre-emptive treatment.
