Paratesticular rhabdomyosarcomas (RMS) are very rare malignant tumors arising from the mesenchymal tissues of tunica, epididymis or spermatic cord. They present as painless hard masses in inguinoscrotal region, and large tumors can be mistaken for testicular tumors. They can spread to retroperitoneal lymph nodes or hematogenously metastasize to lung, bones and bone marrow. Here, we report a case of Embryonal RMS with spindle cell component presenting with painless scrotal mass and lung metastasis at initial diagnosis.

Abstract Background: Thyroid cancer (TC) is common malignancy. Lung metastasis is one of the top metastases for TC. The incidence and survival rates of TC with lung metastasis remains unclear.Methods: Data on TC with lung metastasis and other site-specific metastases were extracted from the Surveillance, Epidemiology, and End Results (SEER) database. The chi-square tests were employed to compare the clinicopathological characteristics among patients with different sites of metastases. Kaplan-Meier analysis and log-rank tests were used for survival analysis. A Cox proportional model was used for multivariate analyses of the patient population. Statistical significance indicated by a two-tailed P value < 0.05. Results: A total of 77322 patients with TC and known sites of distant metastases were identified from 2010-2016. The probability of isolated lung metastasis was significantly higher than that of isolated distant metastasis to other sites among TC patients (P < 0.05). Patients with isolated lung metastases had worse overall and thyroid cancer-specific survival compared to patients with isolated bone metastases (P < 0.05). There was a slight difference in thyroid cancer-specific survival between patients with lung metastasis and patients with liver metastasis(P=0.0496), while there was no significant difference in overall survival. (P >0.05). There was no significant difference in overall survival or thyroid cancer specific survival between patients with lung metastasis and those with brain metastasis (P > 0.05). Multivariate analysis revealed that white race was associated with better outcomes in terms of both endpoints in the lung metastasis population.Conclusions: The incidence of lung metastasis from TC was higher than that of other organ metastases. Thyroid cancer patients with isolated lung metastases have worse outcomes compared to patients with isolated bone metastases and liver metastases, whereas is similar to brain metastasis. There was the worst survival outcome on patients with multi-organ metastases.

Aims and background Surgical resection remains the mainstay for the treatment of colorectal lung metastasis, but a group of patients who are medically inoperable or unsuitable for surgery are treated with radiotherapy. The purpose of this multi-institutional study was to evaluate the clinical outcome and investigate the prognostic factors affecting local control and survival in this subset of patients. Methods We retrospectively analyzed 30 patients with 43 lesions who underwent curative radiotherapy for isolated lung metastasis from colorectal cancer at nine institutions from 2003 and 2008. A total dose of 42–75 Gy at the peripheral planning target volume was administered in 3–35 fractions. The median biologically equivalent dose was 84 Gy (range, 58.5–180). Results Treatment response was complete in 10 (33.3%), partial in 13 (43.3%), stable in six (20.0%), and progressive in one patient (3.3%). The median follow-up period for all patients was 29.0 months (range, 5.0–93.8). Kaplan-Meier local control at 5 years was 44%. The median survival was 46.2 months, and the 5-year overall survival was 47%. Twenty-three patients (77%) experienced treatment failure, most of which were intrapulmonary failure. The intrapulmonary relapse-free survival and overall relapse-free survival at 5 years were 22% and 19%, respectively. Treatment response and pre-radiotherapy carcinoembryonic antigen level were significant prognostic factors for local control and survival. Grade 3–5 toxicity occurred in 7 patients. Three patients had grade 5 toxicity, including radiation pneumonitis, a tracheoesophageal fistula, and hemoptysis. Conclusions Curative radiotherapy for isolated lung metastasis from colorectal cancer in patients who are medially inoperable or unsuitable for surgery results in long-term survival, comparable to surgical resection. Curative radiotherapy could be an effective and noninvasive alternative if dose-limiting toxicity is carefully considered, particularly in patients with bilateral or central lesions.
