Assessing change in quality of life using the Oral Health Impact Profile (OHIP) in patients with different dentofacial deformities undergoing orthognathic surgery: a before and after comparison

Abstract Objective The objective of this study was to assess the impact of orthognathic surgery for dental facial deformities on oral health-related quality of life (OHRQoL) in the immediate postoperative period up to at least 1 year after surgery. Study design This prospective study evaluated data from 85 patients. OHRQoL was assessed using the Dutch version of the Oral Health Impact Profile questionnaire (OHIP-14NL) preoperatively (T0), each day for 7 days postoperatively (T1–T7) and 4 weeks (T8), 6 months (T9), and at least 1 year (T10) after surgery. The total OHIP score was calculated for each patient, with higher OHIP scores indicating a worse impact on oral health. Patients also completed an extra questionnaire about self-care, discomfort, and experienced pain (rated on a 10-point scale) in the postoperative period (T1–T10). Results The mean OHIP score increased sharply at T1 compared to T0 but decreased significantly in the first postoperative week. The mean OHIP score at T8 was still higher than before surgery. However, at T9 and T10, the mean OHIP score was significantly lower than at T0 (P < .05). No significant difference in OHIP score was found between gender, age, type of surgery, and indication for surgery. Pain significantly decreased from T6 to T0. The OHIP and pain scores significantly positively correlated at every time point except T9. Conclusion The findings indicate that OHRQoL is reduced from baseline in the immediate postoperative period but improves over time. By 1 year, OHRQoL improves significantly after orthognathic surgery in patients with dentofacial deformities.

ABSTRACT This case study aims to describe the effects of orthognathic surgery on the quality of life of a patient with class III dentofacial deformity. The protocols Oral Health Impact Profile, short version, and Orthognathic Quality of life Questionnaire were applied before and after orthognathic surgery to measure the quality of life of a subject. The data were collected from a clinical case report. We observed that, by the Oral Health Impact Profile, the quality of life was not affected by dentofacial deformity (score = 1), and that there was a 30-day postoperative score increase (score = 9), indicating a worsening of quality of life and an improvement after 90 days (score = 4). On the other hand, by the Orthognathic Quality of life Questionnaire, it was noticed that dentofacial deformity negatively affected the patient's quality of life (score = 34), and that there was a progressive improvement in scores after surgery, i.e., from score 15 to 2, showing a positive impact on quality of life. The orthognathic surgery positively influenced the patient's quality of life, requiring the application of specific protocols that accurately measure the impacts of the procedure on the individual's life.

Background: The aim of the study was to compare the oral-health-related quality of life (OHRQoL) between cancer survivors: with rapid orthodontic treatment (TX) (up to 12 months) and standard TX (orthodontic treatment time longer than 16 months). Methods: There were 76 cancer survivors (48 women and 28 men) allocated into groups with rapid (36 people) or standard (40 people) duration of TX. OHRQoL was assessed on the basis of Oral Health Impact Profile (OHIP-14) values, measured before TX, 2 weeks and 3 months after the onset of TX, and after the end of TX. A repeated ANOVA test was used to check the statistical significance between the scores. Results: There were no significant differences regarding the OHIP-14 values between the examined groups at all stages of the examination. Both groups presented significant (p < 0.001) improvement of the values of OHRQoL at the end of TX comparing to the values achieved before the onset of TX. Conclusions: Duration of orthodontic treatment by itself had no impact on oral-health-related quality of life.

ABSTRACT Objectives (1) To assess the effectiveness of the Orthognathic Quality of Life Questionnaire (OQLQ) and the Child Oral Health Impact Profile (COHIP) to detect differences in Oral Health-Related Quality of Life (OHRQoL) between pediatric patients with dentofacial deformities and controls. (2) To assess for correlations between scores from the OQLQ and COHIP domains with the type and severity of the skeletal mal-relationship. (3) To assess if the COHIP and OQLQ were identifying unique or overlapping OHRQoL concerns. Materials and Methods Subjects were under age 18, presented with a dentofacial deformity, and completed both surveys. Matched controls completed the same. Severity for conditions was recorded via overjet, overbite, and ANB values and subjects were classified as skeletal Class I, II, or III. Results Enrollment yielded 30 subjects and 31 controls. For the OQLQ, significant differences between subjects and controls were found for the Facial Esthetics domain, Oral Function domain, and total score. For the COHIP, significant differences were found for the Social/Emotional Well-Being and Self-Image domains plus total score. There were no significant correlations between the severity of the condition as measured by overjet and reported OHRQoL for any domains. Conclusions The OQLQ and COHIP are effective at detecting significant OHRQoL differences between pediatric patients with dentofacial deformities and controls. Although there is some overlap in the results, the instruments appear to identify different OHRQoL concerns.

Με τη συγκεκριμένη έρευνα, επιχειρείται μία σχοινοτενής διερεύνηση και πραγμάτευση της συμπεριφοράς των ατόμων με αναπηρία και των φροντιστών τους (μαθητών, γονέων, κηδεμόνων, εκπαιδευτικών) απέναντι στην προτεινόμενη οδοντιατρική θεραπεία. Εν προκειμένω, καθίσταται μία προσπάθεια ιχνηλάτησης και κατανόησης των μεταβλητών, που επενεργούν καταλυτικά στη στάση και συμπεριφορά των ατόμων με αναπηρία και των φροντιστών τους. Στην εμπειρική έρευνα που διεξήχθη τα σχολικά έτη 2018-2019 και 2019-2020 έλαβαν μέρος εκατόν ενενήντα επτά (197) άτομα (μαθητές, γονείς, κηδεμόνες, νηπιαγωγοί, δάσκαλοι, καθηγητές) που αποτελούν μέλη λειτουργίας των ΣΜΕΑΕ («Ειδικά Νηπιαγωγεία, Ειδικά Δημοτικά, Ειδικά Γυμνάσια και Λύκεια και Ε.Ε.Ε.Ε.Κ. (Εργαστήρια Ειδικής Επαγγελματικής Εκπαίδευσης και Κατάρτισης»). Ως εργαλείο έρευνας χρησιμοποιήθηκε το ερωτηματολόγιο, το οποίο σχεδιάστηκε, συντάχθηκε και εκπονήθηκε από τον ερευνητή στη λογική αντίστοιχων διεθνών (Oral Health Impact Profile, Child Oral Health Quality of Life, Parental-Caregiver Perceptions Questionnaires, Oral Health Index) που δίνονται προς συμπλήρωση στους φροντιστές των ατόμων με αναπηρία, όταν καταφεύγουν στις οδοντιατρικές μονάδες και κλινικές, προκειμένου να εξεταστούν ώστε να ακολουθήσουν την προτεινόμενη οδοντιατρική θεραπεία. Τα εμπειρικά ευρήματα και εκβάσεις, καταληκτικά, κατάδειξαν ότι η κακή κατάσταση της στοματικής κοιλότητας των ατόμων με αναπηρία, επηρεάζει ελάχιστα τη γνωστική τους απόδοση, τον ύπνο τους, τη μάσηση των τροφών και το συναισθηματικό και συμπεριφορικό τους προφίλ κατά τη διάρκεια της σχολικής τους καθημερινότητας και ζωής.

ABSTRACT Aim This paper describes a case of an eruption hematoma in a 20-month-old boy and the impact of this hematoma on the quality of life (QoL) related to oral health of this infant and his family. Background Eruption hematoma is a soft benign cyst that contains blood and overlie a tooth that are about to erupt. Oral health conditions can affect the QoL and bring psychological impacts. Case report The proposed treatment was based on oral hygiene instruction, normal diet and massage on the lesion area. A weekly follow-up visits up to the spontaneous regression at the 6 weeks of the lesion was conducted. The impact on QoL was assessed though the Brazilian version of the early childhood Oral Health Impact Scale (B-ECOHIS) questionnaire before and after 2 months of the hematoma regression. Conclusion The presence of eruption hematoma impacted, physically and emotionally, the QoL related to oral health of the child and his family, and this impact decreased when the eruption hematoma disappeared. Clinical significance It is important the knowledge of the dentists about eruption cyst/hematoma to make the correct decisions to improve the QoL of their patients and families.
