Functional mitral regurgitation affects a substantial proportion of patients with congestive heart failure due to myocardial infarction or dilated cardiomyopathy. Functional mitral regurgitation greatly increases morbidity and mortality. Surgical annuloplasty is the standard of care for symptomatic patients with moderate or severe functional mitral regurgitation; however, a large number of patients are refused surgery. Several percutaneous approaches have been developed to address the need for less invasive treatment of mitral annulus dilatation. Devices using coronary sinus to cinch the mitral annulus are relatively easy to use; however, a number of factors may limit their clinical application, such as suboptimal anatomical relationship between the coronary sinus and mitral annulus, risk of coronary artery compression, large variability in the coronary venous anatomy and conflict with other therapies such as ablation or cardiac resynchronisation. Direct mitral annuloplasty is anticipated to be more effective than the coronary sinus approaches; however, it has yet to prove its safety and efficacy in carefully designed clinical trials. The best candidates and the best timing for each percutaneous mitral annuloplasty therapy, whether direct or indirect, have yet to be identified.

Objective This study aimed to review the anatomical and clinical characteristics of internal carotid-posterior communicating artery (IC-PC) aneurysms, especially those located close to the skull base. Methods The microsurgical anatomy around the posterior communicating artery (PComA) was examined in a dry skull and five formalin-fixed human cadaveric heads. The clinical characteristics of 37 patients with 39 IC-PC aneurysms, who were treated microsurgically between April 2008 and July 2016, were retrospectively reviewed. Results The anterior clinoid process (ACP), as well as the anterior petroclinoidal dural fold (APF), which forms part of the oculomotor triangle, are closely related to the origin of the PComA. Among the 39 IC-PC aneurysms, anterior clinoidectomy was performed on 4 (10.3%) and a partial resection of the APF was performed on 2 (5.1%). Both of these aneurysms projected inferior to the tentorium, or at least part of the aneurysm's dome was inferior to the tentorium. Conclusion Proximally located IC-PC aneurysms have an especially close relationship with the ACP and APF. We should be familiar with the anatomical relationship between IC-PC aneurysms and the structures of the skull base to avoid hazardous complications.

The Inter-Pedicular and Inter-Pars distance was measured in a plain AP radiography (X-Ray) of 150 and 75 CT images normal patients between 18- 47 years of age. The aim of the study is to measure the normal Inter-Pedicular and Inter-Pars distance. We found that by studying the anatomical relationship between the inner or medial Pedicular border and the Pars outer or lateral border, gives the Orthopaedic Surgeon a reproducible and consistent guide towards exacting a pedicular screw placing. We found that both X-Ray and CT images shows steady increase in the Ipr and Ipd from L1 to L5, there is a minimal difference from L1-L2 and marked difference seen from L3 to L5, and showing the differences in distances are more in the males, compared to females. The Means of all the groups compared also proves that there is steady raise in the diameter of the IPR and IPD from L1 to L5, where there is dramatical and signicant change in the upward direction, noted from L3 to L5. The mean difference is almost constant from L1to L2. So this study, did essentially to help, establish that, the inner medial border of pedicle, is in near relationship to, the outer lateral border of the Pars-Interarticularis, which helps in establishing the latero-medial entry point for the pedicular screw insertion in the lumbar spine.
